眼睑基底细胞癌手术切除结合皮瓣转移的效果分析

Effect Analysis of Surgical Resection of Patients with Eyelid Basal Cell Carcinoma Combined with Flap Metastasis

目的探讨眼睑基底细胞癌(BCC)手术切除结合皮瓣转移的临床治疗效果。方法选取45例接受手术切除结合皮瓣转移治疗的BCC患者为研究对象。分析治疗效果、患者满意度、术后复发率、治疗安全性、转移情况,以及术前、术后抑制跨膜蛋白16a(Tmem16a)、Ki67增殖指数(Ki67)、整合素连接激酶(ILK)表达情况。结果45例BCC患者术后皮瓣均成活,治疗总有效率为88.89%(40/45),其中显效18例(40.00%),有效22例(48.89%),无效5例(11.11%)。患者治疗总满意度为91.11%(41/45),其中非常满意20例(44.44%),满意21例(46.67%),不满意4例(8.89%)。术后未出现转移、复发现象,皮肤对合良好,未出现移植片挛缩、排斥反应,且无睑外翻、角膜溃疡、暴露性角膜炎等并发症发生。术后Tmem16a、Ki67、ILK阳性表达率均低于术前,差异有统计学意义(P<0.05)。结论手术切除结合皮瓣转移治疗BCC疗效确切,可有效降低Tmem16a、Ki67、ILK阳性表达率,患者满意度较高,安全性好。

Objective To investigate the clinical therapeutic effect of surgical resection of eyelid basal cell carcinoma(BCC)combined with flap metastasis.Methods 45 BCC patients treated with surgical resection combined with flap metastasis were selected.Treatment efficacy,patient satisfaction,postoperative relapse rate,safety of treatment,metastasis,as well as preoperative and postoperative inhibition of transmembrane protein 16a(Tmem16a),Ki67 proliferation index(Ki67),integrin junction kinase(ILK)expression were analyzed.Results All 45 BCC patients survived with a total treatment efficiency of 88.89%(40/45),18 effective(40.00%),22 effective(48.89%%)and 5 ineffective(11.11%).Total satisfaction with patient treatment was 91.11%(41/45),with 20 very satisfied(44.44%),21 satisfied(46.67%)and four dissatisfied(8.89%).After surgery,there was no metastasis,recurrence,good skin combination,no graft contracture,rejection,and no eyelid valgus,corneal ulcer,exposed keratitis and other complications.All postoperative positive Tmem16a,Ki67,ILK expression rates were lower than preoperative and statistically significant(P<0.05).Conclusion Surgical resection combined with the treatment of Tmem16a,Ki67,ILK positive expression,high patient satisfaction and good safety.
