血管内介入术治疗肺癌术后迟发性出血的效果及死亡的影响因素

Effect of Endovascular Intervention on Delayed Bleeding After Lung Cancer Surgery and the Influencing Factors of Death

目的探讨血管内介入术治疗肺癌术后迟发性出血的效果及死亡影响因素。方法选取92例肺癌术后迟发性出血患者为研究对象。患者均行血管内介入术治疗,再根据随访预后结果分为不良组(24例)与良好组(68例),对比两组患者临床资料,探讨介入治疗肺癌迟发性出血患者发生死亡的影响因素。经Logistic回归分析,显示高龄、吸烟、非出血性并发症及再出血是血管内介入术治疗肺癌术后迟发性出血患者发生死亡的高危因素(P>0.05)。结果92例患者中,90例止血成功,2例止血失败,全组介入止血控制率为97.82%(90/92)。不良组年龄、吸烟、肿瘤分期、非出血性并发症及再出血情况占比均高于良好组(P<0.05)。结论血管内介入术对肺癌术后迟发性出血效果较好。影响术后迟发性出血死亡的主要因素为高龄、吸烟、非出血性并发症及再出血。

Objective To investigate the effect of endovascular intervention on delayed bleeding after lung cancer surgery and the risk factors for death.Methods 92 patients with delayed bleeding after lung cancer surgery underwent endovascular intervention were selected.Patients were divided into poor group(n=24)and good group(n=68)according to the prognosis status during the follow-up period.The clinical data of the 2 groups was compared,thereafter,the risk factors of death in patients with delayed bleeding after lung cancer surgery underwent endovascular intervention were screened.Logistic regression analysis showed that advanced age,smoking,non-bleeding complications and rebleeding were high risk factors for death in patients with delayed bleeding after lung cancer surgery underwent endovascular interventional treatment(P>0.05).Results Of the 92 patients,hemostasis was successful in 90 cases and failed in 2 cases.The control rate of interventional hemostasis in the whole group was 97.82%(90/92).The proportions of advanced age,smoking,tumor stage,non-bleeding complications and rebleeding in poor group were higher than those in good group(P<0.05).Conclusions Endovascular interventional surgery has a better effect on delayed bleeding after lung cancer surgery.The main factors for the death of delayed bleeding after surgery are advanced age,smoking,non-bleeding complications and rebleeding.
