胫骨高位截骨术与全膝关节置换术对膝关节骨性关节炎患者Lysholm膝关节评分、关节活动度和术后并发症影响

Effects of High Tibial Osteotomy and Total Knee Replacement on Lysholm Knee Score,Joint Range of Motion and Postoperative Complications in Patients with Osteoarthrosis of Knee

目的探究胫骨高位截骨术与全膝关节置换术对膝关节骨性关节炎患者Lysholm膝关节评分、关节活动度和术后并发症的影响。方法对2019年2月—2020年6月行手术治疗的104例膝关节骨性关节炎的临床资料进行回顾性分析,根据手术方式不同分为研究组54例和对照组50例,研究组行胫骨高位截骨术,对照组行全膝关节置换术。比较两组临床疗效,手术前后Lysholm膝关节评分、关节活动度,以及术后并发症。结果研究组临床总有效率高于对照组(P<0.05)。术后3、6个月,两组Lysholm膝关节评分和关节活动度较术前升高,且研究组高于对照组(P<0.05)。研究组术后并发症总发生率低于对照组(P<0.05)。结论胫骨高位截骨术与全膝关节置换术均能改善膝关节骨性关节炎患者Lysholm膝关节评分和关节活动度,胫骨高位截骨术的临床疗效和改善幅度更为显著,还能减少术后并发症。

Objective To explore the effects of high tibial osteotomy and total knee replacement on Lysholm knee score,joint range of motion(ROM)and postoperative complications in patients with osteoarthrosis of knee.Methods Clinical data of 104 patients with osteoarthrosis of knee who had undergone surgical treatments between February 2019 and June 2020 was retrospectively analyzed.The patients were divided into research group(n=54)and control group(n=50)according to different surgical methods.High tibial osteotomy was performed in research group,while total knee replacement was performed in control group.Clinical efficacy,Lysholm knee scores,joint ROM and postoperative complications before and after operation were compared between two groups.Results The total effective rate in research group was significantly higher than that in control group(P<0.05).After operation for 3 months and 6 months,values of Lysholm knee scores and joint ROM were significantly increased compared with those before operation in two groups,and the values in research group were significantly higher than those in control group(P<0.05).The total incidence rate of complications in research group was significantly lower than that in control group(P<0.05).Conclusion Both high tibial osteotomy and total knee arthroplasty may improve the Lysholm knee joint score and joint ROM.Clinical efficacy and improvement condition by high tibial osteotomy are more significant,which may reduce the incidence rate of postoperative complications.
