颅内动脉瘤患者手术前后血清miR-126、miR-125b表达变化及其临床意义

Changes of Serum miR-126 and miR-125b in Patients with Intracranial Aneurysm before and after Surgery and Their Clinical Significances

目的分析颅内动脉瘤患者手术前后血清miR-126、miR-125b表达变化及其临床意义。方法收集2018年8月—2020年8月行手术治疗的104例颅内动脉瘤的临床资料,设为观察组,另选取同期体检的健康者100例为对照组。比较对照组及观察组手术前后血清miR-126、miR-125b表达及基质金属蛋白酶-9(MMP-9)、血管内皮生长因子(VEGF)水平。对观察组进行6个月的随访,根据不同预后分为生存组86例和死亡组18例,并比较术后血清miR-126、miR-125b表达水平。观察术前血清miR-126、miR-125b表达与患者临床特征的关系,同时分析二者与MMP-9、VEGF的相关性。结果观察组术前和术后血清miR-126表达、VEGF、MMP-9水平高于对照组,miR-125b表达低于对照组(P<0.05);观察组术后miR-126表达、VEGF、MMP-9水平低于术前,miR-125b表达高于术前(P<0.05)。术前血清miR-126、miR-125b表达与动脉瘤数目、动脉瘤直径相关(P<0.05,P<0.01)。生存组术后血清miR-126表达低于死亡组,miR-125b表达高于死亡组(P<0.01)。术前血清miR-126表达与VEGF、MMP-9呈正相关(P<0.01),miR-125b表达与VEGF、MMP-9呈负相关(P<0.01)。结论血清miR-126、miR-125b在颅内动脉瘤患者中存在异常表达,与动脉瘤数目、直径及预后有关,且与VEGF、MMP-9具有显著相关性。

Objective To analyze changes of serum miR-126 and miR-125b in patients with intracranial aneurysm(ICA)before and after surgery and their clinical significances.Methods Clinical data of 104 patients with ICA who had undergone surgical treatments between August 2018 and August 2020 was collected,and the patients were included in observation group.Another100 healthy individuals who had undergone physical examinations at the same period were selected as control group.Expressions of serum miR-126 and miR-125b,and levels of matrix metalloproteinase 9(MMP-9)and vascular endothelial growth factor(VEGF)before and after surgery were compared between two groups.The patients in observation group were followed up for 6 months,and they were divided into survival subgroup(n=86)and death subgroup(n=18)according to different prognoses,and expressions of serum miR-126 and miR-125b after surgery were compared between two subgroups.Correlations between serum expressions of miR-126 and miR-125b before surgery with clinicopathological features of patients were observed,and correlations of the two indexes with MMP-9 and VEGF were analyzed.Results Compared with those in control group,serum miR-126 expressions,and levels of VEGF and MMP-9 were significantly higher,while miR-125b expressions were significantly lower before and after surgery in observation group(P<0.05).Compared with those before surgery,miR-126 expression,and levels of VEGF and MMP-9 were significantly lower,while miR-125b expression was significantly higher after surgery in observation group(P<0.05).The preoperative expressions of serum miR-126 and miR-125b were related to number and diameter of aneurysms(P<0.05,P<0.01).Compared with those in death subgroup,postoperative expression of serum miR-126 was significantly lower,while expression of serum miR-125b was significantly higher in survival subgroup(P<0.01).Before surgery,expression of serum miR-126 was positively correlated with VEGF and MMP-9(P<0.01),while expression of serum miR-125b was negatively correlated with VEGF and MMP-9(P<0.01).Conclusion Serum miR-126 and miR-125b are abnormally expressed in patients with ICA,which are related to the number,diameter and prognosis of ICA.Meanwhile,there are significant correlations between the two indexes with VEGF and MMP-9.
