急诊抢救室急性上消化道出血患者1214例相关临床特征

Related Clinical Features of Acute Upper Gastrointestinal Bleeding in Emergency Department:Analysis of 1214 Cases

目的了解急诊抢救室急性上消化道出血(acute upper gastrointestinal bleeding,AUGIB)患者相关临床特征。方法回顾性分析急诊抢救室1214例AUGIB患者人口学特征、就诊时间分布、抢救时间以及转归等流行病学资料。结果1214例急诊抢救室AUGIB患者中,男女比例为2.32∶1,老年组患者占比47.37%,性别及年龄分布之间差异有统计学意义(P<0.001)。就诊量最高的月份依次是1、4、3、10月,最低月份为2、7月;圆形分布显示急诊抢救室AUGIB高峰时日为1月8日,高峰时段为10月5日至次年4月11日;每日AUGIB就诊量在22:00~00:00最多,高峰时间点为19:13,高峰时段为12:21至次日02:05。所有患者中,0例死亡,80例好转出院(抢救时间439.6 min),823例转入普通病房(抢救时间352.2 min),242例转入ICU(抢救时间138.0 min),69例自动离院(抢救时间521.5 min)。结论本研究揭示了急诊抢救室AUGIB患者的人群特征、就诊时间分布特点以及抢救时间、转归情况,为优化AUGIB急诊流程及合理使用急诊资源提供参考。

Objective To analyze the related clinical features of patients with acute upper gastrointestinal bleeding(AUGIB)in emergency department.Methods Clinical data of 1214 patients with AUGIB were collected.The demographic characteristics,distribution of emergency visit time,rescue time,and outcomes of patients were analyzed retrospectively.Results Among 1214 AUGIB cases,the male-female ratio was 2.32∶1,patients over 60 years old accounted for the most(47.37%).The differences in gender and age distribution were statistically significant(P<0.001).The emergency visits were mainly distributed in January,April,March,and October,and the lower months were in February and July.The circular distribution showed that the peak time were January 8 while the peak period of AUGIB in emergency department were from October 5th to April 11st of the following year.The daily UGIB emergency visits were the largest between 22:00~00:00 while the peak time was 19:13.And the period were from 12:21 to 02:05 of the following day.Among all patients,0 case died,80 cases were discharged(rescue time:439.6 minutes),823 cases were transferred to general ward(rescue time:352.2 minutes),242 cases were transferred to ICU(rescue time:138.0 minutes),and 69 cases were discharged automatically(rescue time:521.5 minutes).Conclusion This study revealed the demographic characteristics,distribution of emergency visit time,rescue time,and outcomes of patients in our emergency department,which will provide valuable knowledge in optimizing AUGIB emergency procedures and rational use of emergency resources.
