乳腺癌化疗患者经外周静脉穿刺的中心静脉导管置管中导管异位的影响因素

Risk factors of ectopic peripherally inserted central venous catheter in breast cancer patients undergoing chemotherapy

目的探讨乳腺癌化疗患者经外周静脉穿刺的中心静脉导管(PICC)置管中导管异位的影响因素。方法选取210例行PICC置管乳腺癌患者,分析发生和未发生导管异位患者临床特征的差异,并分析影响因素。结果 210例行PICC置管乳腺癌患者中,发生导管异位25例,导管异位发生率为11.90%。发生导管异位患者中年龄﹤60岁、体重指数≥24 kg/m^(2)、置管静脉为肘正中静脉/头静脉、送管次数≥2次、置管时间≥6个月比例均高于未发生导管异位患者,差异均有统计学意义(P﹤0.05)。Logistic回归分析结果显示,年龄、体重指数、置管静脉、置管时间是乳腺癌患者PICC置管发生导管异位的影响因素(OR=0.715、1.510、2.406、2.128,P﹤0.05)。结论乳腺癌化疗患者PICC置管导管异位发生与患者年龄、体重指数、置管静脉及置管时间有关,临床上应加以重视。

Objective To investigate the risk factors of the ectopic peripherally inserted central venous catheter(PICC)in breast cancer patients undergoing chemotherapy Method A total of 210 breast cancer patients who underwent PICC catheterization were selected.The differences in clinical characteristics between patients with and without ectopic PICC were analyzed,and the risk factors were also concluded.Result Among 210 breast cancer patients who underwent PICC catheterization,25 cases experienced ectopic PICC,and the incidence of this event was 11.90%.The proportion of patients with ectopic PICC age<60 years old,body mass index(BMI)≥24 kg/m^(2),median cubit vein/cephalic vein,catheter delivery times≥2,and catheterization time≥6 months were higher than those without ectopic PICC,the differences were statistically significant(P<0.05).Logistic regression analysis showed that age,BMI,catheterization vein,and catheterization time were the risk factors of ectopic PICC in breast cancer patients(OR=0.715,1.510,2.406,2.128,P<0.05).Conclusion The incidence of ectopic PICC catheterization in breast cancer chemotherapy patients is related to their age,BMI,catheterization vein,and catheterization time.The conclusion should raise great concern in clinical practice.
