颈动脉支架置入术中栓塞保护装置卡压一例

A case of embolic protection device entrapment during carotid artery stenting

栓塞保护装置(EPD)能够有效减少颈动脉支架置入术(CAS)术中栓塞的风险,目前在术中常规使用。EPD卡压是CAS术中较为罕见的意外事件,避免此类事件的发生可以提高手术的安全性。该文报道1例CAS术中可能由于操作不当导致的EPD被支架卡压的病例,术者术中反复回收EPD失败后导致EPD导丝断裂,最后通过再次置入1枚支架解决了该术中并发症。作者分析了出现该手术中并发症的原因,并对处理此类并发症的方法提出建议,以供临床医师参考。

Embolic protection device(EPD)can reduce the risk of embolization during carotid stenting(CAS)and is currently routinely used during operation.EPD entrapment is a relatively rare intraoperative incident and avoiding such event can improve the safety of surgery.The authors reported one case of EPD entrapment by the stent may due to improper operation during CAS.The EPD guide wire fractured after failure of repeated attempts of EPD retrieval,and finally the operator solved the intraoperative complication with replacement of another stent.The authors analyzed the causes of this complication and suggested related management to provide reference to clinicians.
