愈胃方联合替普瑞酮治疗萎缩性胃炎肠化生的临床研究

Clinical Study on Yuwei Prescription Combined with Teprenone in the Treatment of Intestinal Metaplasia of Atrophic Gastritis

目的自拟中药方剂“愈胃方”联合替普瑞酮治疗萎缩性胃炎、肠化生的临床研究。方法选取2019年1月—2021年2月寿光市人民医院门诊收治的有纳差、上腹部不适,经胃镜、组织病理学检查为萎缩性胃炎(木村-竹本分级C1~C3,O1~O3)的50例患者,随机分为2组。对患者进行幽门螺杆菌清除治疗后,对照组给予标准剂量替普瑞酮治疗2月,治疗组在此基础上给予自拟药物“愈胃方”,每日2次;采用胃黏膜萎缩程度分期标准(Operative link for gastritis assessment,OLGA)及胃黏膜肠化生分期标准(Operative link for gastric intestinal mataplasia assessment,OLGIM)评估治疗效果。结果对照组与治疗组均能有效减轻萎缩性胃炎、肠化生的程度;治疗后症状缓解率、胃镜下表现均发生明显变化,替普瑞酮加中药的治疗效果要优于单替普瑞酮组,且差异有统计学意义(P<0.05)。结论抗幽门螺杆菌后采用自拟“愈胃方”联合替普瑞酮治疗萎缩性胃炎、肠化生可获得较好效果。

Objective To study the clinical effect of Yuwei prescription combined with teprenone in the treatment of atrophic gastritis and intestinal metaplasia.Methods 50 patients with anorexia and epigastric discomfort in Shouguang People’s Hospital were selected.All patients were diagnosed as atrophic gastritis(Kimura-Takemoto classification C1-C3,O1-O3)by gastroscopy and histopathology.They were divided into two groups.The control group was treated with teprenone for 2 months,and the treatment group was given Yuwei prescription on the basis of previous treatment.Operative link for Gastrectomy assessment was used to serve as a plaque for gastric mucosa.OLGA)and Operative link for gastric mucosal mataplasia assessment(OLGIM).Results The degree of atrophic gastritis and intestinal metaplasia could be effectively alleviated in both the control group and the treatment group.Symptom relief rate and gastroscopic manifestations changed significantly after treatment,and the treatment effect of tiprexone plus Chinese medicine was better than that of teprenone alone,and the difference was statistically significant(P<0.05).Conclusion Yuwei prescription combined with teprenone has a good effect on atrophic gastritis and intestinal metaplasia.
