血必净注射液对老年脓毒症休克患者乳酸清除率的影响

Impact of Xuebijing injection on lactate clearance rate in elderly patients with septic shock

目的观察血必净注射液对老年脓毒症休克患者乳酸清除率及预后的影响。方法收集2017年6月至2020年5月山东省泰山医院重症医学科(ICU)收治的90例老年脓毒症休克患者,根据转入ICU先后顺序将患者分为对照组(常规治疗组)和治疗组(常规治疗加血必净注射液治疗组),计算6 h、12 h、24 h、48 h乳酸清除率,并计算第1、2、3、7天急性生理与慢性健康评分(APACHEⅡ评分)及序贯器官衰竭评分(SOFA评分)及28 d死亡率。并使用SPSS 22.0统计学软件对数据进行分析。结果治疗组的APACHEⅡ及SOFA评分均比对照组降低,差异有统计学意义(P<0.05);治疗组的乳酸清除率较对照组明显提高(P<0.05);治疗组与对照组比较28 d死亡率明显下降,差异有统计学意义(P<0.05)。结论血必净注射液可以提高老年脓毒症休克患者乳酸清除率、降低死亡率、改善预后。

Objective To observe the impact of Xuebijing injection on lactate clearance rate and prognosis in elderly patients with septic shock.Methods A total of 90 elderly patients with septic shock admitted to ICU of Department of Intensive Care Unit,Shandong Provincial Taishan Hospital from June 2017 to May 2020 were collected,and they were divided into the control group(the conventional treatment group)and the treatment group(the conventional treatment combined with Xuebijing injection group)according to the sequence of transfer to ICU.The lactate clearance rates at 6 h,12 h,24 h and 48 h were calculated,and the acute physiological and chronic health score(APACHEⅡscore),sequential organ failure score(SOFA score)on the 1st,2nd,3rd and 7th day and 28-day morality were calculated.Meanwhile,SPSS 22.0 statistical software was used to analyze the data.Results The APACHEⅡand SOFA scores in the treatment group were both lower than those in the control group,with statistically significant differences(P<0.05).The lactate clearance rate in the treatment group was significantly higher than that in the control group(P<0.05).Compared with the control group,the 28-day mortality of the treatment group decreased significantly,with statistically significant difference(P<0.05).Conclusion Xuebijing injection can improve lactate clearance rate,reduce mortality,and improve prognosis in elde rly patients with septic shock.
