急性进展性缺血性脑卒中与急性非进展性缺血性脑卒中的代谢特征差异

Differences in metabolic profiles between acute progressive ischemic stroke and acute non-progressive ischemic stroke

目的:探讨急性进展性缺血性脑卒中(acute progressive ischemic stroke,APIS)与急性非进展性缺血性脑卒中(acute non-progressive ischemic stroke,ANPIS)患者血清代谢组的差异。方法:选取15例APIS患者和25例ANPIS患者为研究对象,利用气相色谱质谱联用仪检测两组患者血清代谢组,比较其中的差异。结果:偏最小二乘法判别分析显示APIS患者与ANPIS患者血清的代谢轮廓存在差异。在血清中共筛选到8种差异代谢物,其中在APIS患者中上调的5种,包括2种氨基酸(苏氨酸、丝氨酸)与3种脂肪酸(顺式棕榈酸、棕榈酸、反油酸);下调的代谢物有3种,包括2种氨基酸(脯氨酸、谷氨酰胺)与1种核苷酸前体(次黄嘌呤)。这些代谢物所反映的代谢通路有甘氨酸与丝氨酸代谢、脯氨酸代谢、甘油酯代谢、脂肪酸代谢、嘌呤代谢与鸟氨酸循环、磷脂酰乙醇胺合成代谢等。结论:APIS与ANPIS患者的血清代谢组差异明显,存在与APIS发生发展相关的代谢改变。

Objective:To investigate the differences in serum metabolome between patients with acute progressive ischemic stroke(APIS)and acute non-progressive ischemic stroke(ANPIS).Methods:15 patients with APIS and 25 patients with ANPIS were selected for the study,and the serum metabolomes of the two groups were examined using gas chromatography-mass spectrometry to compare the differences among them.Results:Partial least squares discriminant analysis revealed differences in the metabolic profiles of APIS patients and ANPIS patients.A total of 8 differential metabolites were screened in serum,of which 5 were upregulated in APIS patients,including 2 amino acids(threonine,serine)and 3 fatty acids(cis-palmitic acid,palmitic acid,transoleic acid);3 metabolites were downregulated,including 2 amino acids(proline,glutamine)and 1 nucleotide precursor(hypoxanthine).The metabolic pathways reflected by these metabolites were glycine and serine metabolism,proline metabolism,glycerol ester metabolism,fatty acid metabolism,purine metabolism and orrdthine cycle,and phosphatidylethanolamine anabolism.Conclusion:The serum metabolome of patients with APIS and ANPIS is significantly different,and there are metabolic changes associated with the development of APIS.
