儿童寰枢椎半脱位牵引治疗效果的影响因素

Influencing factors of traction in the treatment of children with atlantoaxial rotatory subluxation

目的分析儿童寰枢椎半脱位行枕颌带牵引治疗效果的影响因素。方法分析2015年9月~2020年10月行枕颌带牵引治疗的寰枢椎半脱位患儿65例,收集其临床资料,分析患儿的Fielding分型、年龄、性别、诱因、症状持续时间、牵引前有无治疗等因素,以牵引所需要的时间作为治疗效果的量化指标,经多元线性回归分析找出可能影响牵引治疗效果的相关因素。结果63例患儿牵引治疗效果显著。经多元线性回归分析,症状持续时间和Fielding分型对治疗有效时间有显著影响(P<0.05)。结论Fielding分型越低,症状持续时间越短,则牵引治疗时间越少、效果越好。

Objective To analyze the influencing factors of Jaw-occipital belt traction in the treatment of children with atlantoaxial rotatory subluxation.Methods Sixty-five children with atlantoaxial rotatory subluxation who underwent Jaw-occipital belt traction from September 2015 to October 2020 were analyzed.The clinical data were collected and the factors such as Fielding classification,age,gender,inductive factors,duration of symptoms,and presence or absence of treatment before traction were analyzed.The time for traction was used as a quantitative indicator of treatment effect.Multiple linear regression analysis was performed to find out the related factors that may affect the effect of traction treatment.Results The clinical effect was significant in 63 children with traction treatment.Multiple linear regression analysis showed that symptom duration and Fielding classification had significant influence on effective time of treatment(P<0.05).Conclusion The lower the Fielding classification or the shorter the duration of symptoms,the less the traction treatment time and the better the treatment effect.
