纤维肌痛与腰椎后路融合术后并发症、再入院率和住院费用的相关性

Relationship between fibromyalgia and complications,readmission rate and increased hospitalization cost after posterior lumbar fusion

目的探讨纤维肌痛与后路腰椎融合术后并发症、再入院率和住院费用增加的相关性。方法选择2016年6月~2019年5月在本院接受脊柱融合术的87例纤维肌痛综合征(fibromyalgia syndrome,FMS)患者作为研究组,按1∶1比例抽取同期在本院接受后路减压植骨融合内固定术的非FMS患者87例作为对照组。观察两组患者术后并发症、住院时间、住院费用和再入院率。结果两组患者深静脉血栓、术后输血、切口并发症、椎间隙感染、神经功能缺失、脑脊液漏等并发症发生率比较,差异均无统计学意义(P>0.05);研究组肺部感染、术后贫血的发生率显著高于对照组,差异有统计学意义(P<0.05)。研究组住院天数和住院费用均显著高于对照组,差异有统计学意义(P<0.05);研究组再入院率高于对照组,但差异无统计学意义(P>0.05)。结论纤维肌痛与后路腰椎融合术患者术后贫血和肺炎发生增加相关,与住院费用和住院时间增加相关,但未增加患者再入院率。

Objective To explore the correlation between fibromyalgia and complications,readmission rate and increased hospitalization cost after posterior lumbar fusion.Methods From June 2016 to May 2019,87 patients with fibromyalgia syndrome(FMS)who received spinal fusion in our hospital were selected as the study group,and 87 non-FMS patients who received posterior decompression,bone grafting and internal fixation in our hospital at the same time were selected as the control group according to the ratio of 1:1.The complications,hospitalization time,hospitalization cost and readmission rate were observed.Results There were no significant differences between the two groups in the incidence rates of deep vein thrombosis,postoperative blood transfusion,incision complications,intervertebral infection,neurological deficit,CSF leakage and other complications(P>0.05).The incidence rate of pulmonary infection and anemia in the study group was higher than that in the control group(P<0.05).The hospitalization days and expenses of the study group were higher than those of the control group,the differences were statistically significant(P<0.05).The readmission rate of the study group was higher than that of the control group,but the difference was not statistically significant(P>0.05).Conclusion Fibromyalgia is related to the increased incidence of anemia and pneumonia in patients with posterior lumbar fusion.It is related to the increase of hospitalization cost and hospitalization time,but not related to the increase of readmission rate.
