清宫正骨手法联合骶管注射治疗腰椎间盘突出症的临床疗效观察

Clinical effect of Qinggong Zhenggu manipulation combined with sacral canal injection in the treatment of lumbar disc herniation

目的观察清宫正骨手法联合骶管注射治疗腰椎间盘突出症(lumbar disk herniation,LDH)的临床疗效。方法将90例LDH患者随机分为手法骶管注射组、牵引骶管注射组、骶管注射组(对照组)各30例。手法骶管注射组采用清宫正骨手法联合骶管注射治疗;牵引骶管注射组采用腰椎牵引联合骶管注射治疗;对照组采用骶管注射治疗,比较三组的JOA、VAS评分和直腿抬高角度、疗效。结果治疗后三组患者的JOA评分均显著高于治疗前(P<0.05),VAS评分均低于治疗前(P<0.05),直腿抬高角度显著高于治疗前(P<0.05);治疗后三组间比较,手法骶管注射组的JOA评分>牵引骶管注射组>对照组(P<0.05),手法骶管注射组、牵引骶管注射组的直腿抬高角度和总有效率均显著高于对照组(P<0.05)、VAS评分显著低于对照组(P<0.05)。结论清宫正骨手法联合骶管注射能更有效地改善患者症状、恢复其功能,提高生活质量。

Objective To observe the clinical effect of Qinggong Zhenggu manipulation combined with sacral canal injection in the treatment of lumbar disc herniation(LDH).Methods A total of 90 patients with LDH were randomly divided into manual sacral injection group,traction sacral injection group and sacral injection group(control group).Manual sacral canal injection group was treated with Qinggong Zhenggu manipulation combined with sacral canal injection.The traction sacral canal injection group was treated with lumbar traction combined with sacral canal injection.The control group was treated with sacral canal injection.The JOA,VAS scores,straight leg elevation angle and curative effect of the three groups were compared.Results After treatment,the JOA score of the three groups were significantly higher than before treatment(P<0.05),the VAS score was lower than before treatment(P<0.05),and the straight leg elevation angle was significantly higher than before treatment(P<0.05).After treatment,the JOA score of manual sacral injection group was higher than that of traction sacral injection group and control group(P<0.05).The straight leg elevation angle and total effective rate of manual sacral injection group and traction sacral injection group were significantly higher than those of control group(P<0.05),and the VAS score was significantly lower than that of control group(P<0.05).Conclusion Qinggong Zhenggu manipulation combined with sacral canal injection can effectively improve patients'symptoms,restore their function and improve the quality of life.
