新鲜胚胎移植周期参数对冻融胚胎移植周期妊娠结局的影响 被引量:1

摘要 目的研究新鲜胚胎移植周期参数对冻融胚胎移植(FET)周期妊娠结局的影响。方法采用回顾性队列研究,包括2014年1月至2018年12月间在我院生殖医学科进行的84个FET周期的数据。主要终点为FET周期妊娠率。结果共分析84个FET周期对应的新鲜胚胎移植周期的所有数据。继发性不孕症患者的妊娠率显著高于原发性不孕症患者(分别为39.1%和15.8%,P=0.018)。新鲜胚胎移植周期妊娠的患者在FET周期中的妊娠率显著高于新鲜胚胎移植周期未妊娠的患者(分别为56.3%和11.5%,P<0.001)。冷冻胚胎的数量<5个的患者妊娠率高于冷冻胚胎数目为5~9个和≥10个的患者(分别为47.8%、20.0%和7.7%,P=0.006)。冷冻复苏间隔>12个月患者的妊娠率高于冷冻复苏间隔≤12个月患者(分别为43.3%和20.4%,P=0.025)。结论新鲜胚胎移植周期妊娠是FET周期妊娠成功的良好预后因素。延迟FET超过12个月可能会获得更好的妊娠率。 Objective To investigate the effect of the parameters of fresh embryo transfer cycle on the pregnancy outcome of frozen-thawed embryo transfer(FET)cycle.Methods This retrospective cohort study included data from 84 FET cycles.The primary endpoint was the pregnancy rate in FET cycles.Results The data of 84 FET cycles corresponding to fresh embryo transfer cycles were analyzed.The pregnancy rate of patients with secondary infertility was significantly higher than that of patients with primary infertility(39.1%vs 15.8%,P=0.018).The pregnancy rate during the FET cycle was significantly higher in patients who were pregnant during the fresh embryo transfer cycle than in those who were not pregnant during the fresh embryo transfer cycle(56.3%vs 11.5%,P<0.001).The pregnancy rate of patients with less than 5 frozen embryos was higher than that of patients with 5 to 9 frozen embryos and those with more than 10 frozen embryos(47.8%vs 20.0%and 7.7%,P=0.006).The pregnancy rate of patients with freezing-resuscitation interval>12 months was higher than that of patients with freezing-resuscitation interval≤12 months(43.3%vs 20.4%,P=0.025).Conclusion Pregnancy in patients undergoing fresh embryo transfer cycles is a good prognostic factor for the success of FET cycle.Delaying FET for more than 12 months may result in a better pregnancy rate.
