创伤性骨折患者血浆D-二聚体水平表达及其临床意义

Expression and clinical significance of plasma D-dimer in patients with traumatic fracture

目的分析创伤性骨折患者血浆D-二聚体(D-dimer,D-D)水平表达及其临床意义,旨在为评判创伤性骨折患者病情程度及预后效果提供理论依据。方法选取本院2018年12月至2019年12月收治的78例创伤性骨折患者作为研究对象,按照创伤程度分为轻伤组(n=30)、重伤组(n=26)与危重伤组(n=22),另选取同期于本院行健康体检者40名作为对照组。比较4组血浆D-D水平,分析其与骨折创伤程度相关性。结果对照组D-D水平为(185.23±63.25)μg/L,轻伤组D-D水平为(304.52±83.71)μg/L,重伤组D-D水平为(483.76±123.74)μg/L,危重伤组D-D水平为(532.75±136.82)μg/L;危重伤组D-D水平最高,依次为重伤组、轻伤组,对照组水平最低,差异有统计学意义(F=79.842,P<0.001)。经一般线性双变量Person相关性分析检验得知,D-D水平与创伤性骨折患者创伤程度呈正相关(r=0.826,P=0.000)。结论创伤性骨折患者血浆D-D水平与创伤严重程度呈正相关,血浆D-D水平对评判创伤性骨折患者病情的严重程度和预后具有重要意义,对预估患者病情及预后效果具有较高临床价值。

Objective To analyze the expression of plasma D-dimer(D-D)level in patients with traumatic fractures and its clinical significance,in order to provide theoretical basis for judging the severity and prognosis of patients with traumatic fractures.Methods 78 patients with traumatic fractures admitted to our hospital from December 2018 to December 2019 were selected as the research subjects.According to the degree of trauma,they were divided into mild injury group(n=30),severe injury group(n=26),and critical injury group(n=22),and 40 people who had health checkup in our hospital at the same time were selected as the control group.The plasma D-D levels among the 4 groups were compared,and the correlation between the index value and the degree of fracture trauma was analyzed.Results The DD level of the control group was(185.23±63.25)μg/L,the D-D level of the mild injury group was(304.52±83.71)μg/L,the D-D level of the severe injury group was(483.76±123.74)μg/L,and the D-D level of the critical injury group was(532.75±136.82)μg/L;the critical injury group D-D level was the highest,followed by severe injury group,mild injury group,and the control group has the lowest level,the difference was statistically significant(F=79.842,P<0.001).According to the general linear bivariate Person correlation analysis test,the D-D level is positively correlated with the trauma degree of traumatic fracture patients(r=0.826,P=0.000).Conclusion Plasma D-D levels in patients with traumatic fractures are positively correlated with trauma severity.In clinical practice,plasma DD levels are of great significance in judging the severity and prognosis of patients with traumatic fractures,and are of greater clinical value in estimating their condition and prognosis.
