多系统萎缩患者的尿动力学特点及临床应用

摘要 多系统萎缩(multiple system atrophy,MSA)是一种罕见的神经退行性疾病,早发的和突出的自主神经功能障碍是MSA的重要特征,而神经源性下尿路功能障碍(neurological lower urinary tract dysfunction,NLUTD)又是自主神经损伤的主要表现之一。尿动力学检查是评估NLUTD的可靠方法,不仅能够了解下尿路症状的病理生理机制,也能够发现MSA相对特异的尿动力学表现,在和帕金森病的鉴别诊断和预后评估方面有一定的应用价值。本文将结合MSA自主神经病变的特点对MSA患者尿动力学特点和临床应用进行综述。 Multiple system atrophy(MSA) is a rare neurodegenerative disease characterized by early and prominent autonomic dysfunction. Neurogenic lower urinary tract dysfunction(NLUTD) is one of most frequent categories of autonomic dysfunction. Urodynamic study is a reliable way to evaluate NLUTD. It not only provides insights of pathophysiological changes of the lower urinary tract, but also helps to identify some featured urodynamic changes of MSA.Thus, urodynamic study has potential utility in facilitating the differentiation of MSA and Parkinson’s disease and predicting the prognosis of patients with MSA. This article reviews the urodynamic features and its clinical application in patients with MSA.
