化痰逐瘀散结汤对放化疗非小细胞肺癌免疫功能、肿瘤标记物及疗效的影响

Effects of Huatan Zhuyu Sanjie Decoction(化痰逐瘀散结汤)on Immune Function,Tumor Markers and Efficacy of Chemoradiotherapy for Patients with Non-Small Cell Lung Cancer

目的探讨化痰逐瘀散结汤对放化疗非小细胞肺癌患者免疫功能、肿瘤标记物及疗效的影响。方法选取同步放化疗治疗的非小细胞肺癌患者60例,随机分为对照组及观察组,每组30例,两组患者均采取同步放化疗治疗,化疗方案为PC(顺铂/紫杉醇),观察组患者同时给与化痰逐瘀散结汤口服治疗3周。观察两组患者实际完成疗程情况,两组患者治疗前和治疗后4周时的中医证候积分变化,CD^(+)_(3)、CD^(+)_(4)、CD^(+)_(8)等指标变化,血清癌胚抗原(CEA)、糖类抗原125(CA125)变化。结果两组患者总放疗总剂量及化疗总周期比较差异无统计学意义(P>0.05);两组治疗前神疲乏力、气急、咳嗽、痰血等症状中医证候积分比较差异无统计学意义(P>0.05),治疗后4周时以上症状积分均较治疗前下降,且观察组低于对照组,比较差异有统计学意义(P<0.05);两组治疗前CD^(+)_(3)、CD^(+)_(4)、CD^(+)_(8)水平比较差异无统计学意义(P>0.05),两组治疗后以上指标均较治疗前下降,且对照组低于观察组,比较差异有统计学意义(P<0.05);两组治疗前血清CAE及CA125比较差异无统计学意义(P>0.05),治疗后4周时以上指标水平较治疗前下降,且观察组低于对照组,比较差异有统计学意义(P<0.05);对照组治疗总有效率为76.67%(23/30),观察组为90.00%(27/30),比较差异无统计学意义(P>0.05)。结论同步放化疗治疗的非小细胞肺癌患者给与化痰逐瘀散结汤口服有助于症状、免疫功能的改善,降低血清肿瘤标记物水平,但是对提高治疗疗效证据不足。

Objective To discuss the effects of Huatan Zhuyu Sanjie Decoction(化痰逐瘀散结汤)on immune function,tumor markers and efficacy of chemoradiotherapy for patients with non-small cell lung cancer.Methods We collected 60 cases of patients with chemoradiotherapy for non-small cell lung cancer.The patients were the randomly divided into control group and observation group,30 cases in each group.The patients in both groups were treated with concurrent chemoradiotherapy,and the chemotherapy regimen was PC(cisplatin/paclitaxel).The patients in the observation group were also given the oral treatment of Huatan Zhuyu Sanjie Decoction for 3 weeks.We observed the actual completion of treatment course of the two groups.The changes of TCM symptom scores,CD^(+)_(3),CD^(+)_(4),CD^(+)_(8)and other indicators,serum levels of CEA and carbohydrate antigen 125(CA125)of the two groups before and 4 weeks after treatment.Results There was no statistically significant difference between the two groups in total radiation dose or total chemotherapy cycle(P>0.05).There was no statistically significant difference between the two groups in TCM syndromes scores of fatigue,shortness of breath,cough,sputum and blood before treatment(P>0.05).The scores of above symptoms were 4 weeks after treatment,lower than those before treatment,and the observation group’s scores were lower than those of the control group.The difference was statistically significant(P<0.05).There was no statistically significant difference between the two groups in CD^(+)_(3),CD^(+)_(4)or CD^(+)_(8)before treatment(P>0.05).The above indicators 4 weeks after treatment were lower than those before treatment,and the control group’s were lower than those of the observation group.The difference was statistically significant(P<0.05).There was no statistically significant difference between the two groups in CEA or CA125 before treatment(P>0.05).The above indicators 4 weeks after treatment were lower than those before treatment,and the observation group’s were lower than those of the control group.The difference was statistically significant(P<0.05).The total effective rate was 76.67%(23/30)in the control group and 90.00%(27/30)in the observation group.The difference was not statistically significant(P>0.05).Conclusion The concurrent chemoradiotherapy of non-small cell lung cancer patients with oral administration of Huatan Zhuyu Sanjie Decoction can improve symptoms and immune function and reduce serum tumor markers,but there is insufficient evidence for improving therapeutic efficacy.
