润肺平喘汤加减联合孟鲁司特钠治疗小儿肺脾气虚型慢性咳嗽临床研究

Clinical Study on Modified Runfei Pingchuan Decoction(润肺平喘汤)Combined with Montelukast Sodium in the Treatment of Chronic Cough with Lung Spleen Qi Deficiency in Children

目的观察润肺平喘汤治疗慢性咳嗽的临床疗效。方法选取医院呼吸科门诊收治的慢性咳嗽患儿93例,将所选患儿随机分为两组,对照组46例,观察组47例。对照组患儿给予孟鲁司特钠片治疗,观察组在对照组基础上给予润肺平喘汤治疗。对比两组炎性因子指标、肺功能指标、咳嗽症状评分、中医症候及体征积分及临床疗效。结果观察组总有效率(91.4%,43/47)显著高于对照组总有效率(71.7%,31/46)(P<0.05);两组IL-4、MMP-2、IL-8、MMP-9及TIMP-1水平显著降低(P<0.05),观察组IFN-γ显著升高(P<0.05),并且观察组改善程度较大(P<0.05)。两组FEV1、FVC、FEV1/FVC及PEF水平显著升高(P<0.05),并且观察组升高幅度较大(P<0.05)。观察组治疗后咳嗽评分显著较治疗前高(P<0.05),咳声重浊、咯痰、喉间痰鸣、自汗及纳呆等各项中医症候及体征积分均较治疗前显著降低(P<0.05),并且观察组的各项评分较低(P<0.05)。结论润肺平喘汤治疗慢性咳嗽疗效显著,能够缓解气道炎症反应和抑制气道重塑进展。

Objective To observe the clinical effect of Runfei Pingchuan decoction(润肺平喘汤)on chronic cough.Methods 93 children with chronic cough treated in the outpatient department of respiratory department of the hospital were randomly divided into two groups,46 cases in the control group and 47 cases in the observation group.The children in the control group were treated with montelukast sodium tablets,and the observation group was treated with Runfei Pingchuan decoction on the basis of the control group.The inflammatory factor index,pulmonary function index,cough symptom score,TCM symptom and sign score and clinical curative effect were compared between the two groups.Results the total effective rate of the observation group(91.4%,43/47)was significantly higher than that of the control group(71.7%,31/46)(P<0.05);The levels of IL-4,MMP-2,IL-8,MMP-9 and TIMP-1 decreased significantly in the two groups(P<0.05)γSignificantly increased(P<0.05),and the improvement degree of the observation group was greater(P<0.05).The levels of FEV1,FVC,FEV1/FVC and PEF increased significantly in the two groups(P<0.05),and the increase was greater in the observation group(P<0.05).The cough score of the observation group after treatment was significantly higher than that before treatment(P<0.05),and the scores of TCM symptoms and signs such as severe cough,expectoration,laryngeal phlegm,spontaneous sweating and stupidity were significantly lower than those before treatment(P<0.05),and the scores of the observation group were lower(P<0.05).Conclusion Runfei Pingchuan decoction is effective in the treatment of chronic cough,which can alleviate airway inflammation and inhibit the progress of airway remodeling.
