疫情常态化防控下妇女儿童医院优化门诊全预约诊疗服务中爽约现象的对策

Countermeasures of"break off"in optimizing outpatient full appointment services in women's and children's hospitals under normalization of epidemic prevention and control

目的在新冠疫情常态化防控的当下,优化妇女儿童医院门诊全预约诊疗服务中爽约现象的控制措施,以适应疫情常态化防控工作,减少医疗资源浪费,降低医院管理成本。方法通过HIS后台调出广州市妇女儿童医疗中心2019年1月1日至12月31日新冠疫情前和2020年7月1日至2021年6月30日新冠疫情常态防控期间共9212245例研究对象的门诊就诊数据,对比分析新冠疫情前后爽约情况及当日取消预约情况的变化,采用χ^(2)检验。结果新冠疫情前后预约诊疗服务爽约率分别为3.84%(188942/4918665)和3.78%(162268/4293580),前后比较,差异有统计学意义(P=0.023);疫情前后当日取消预约率分别是2.85%(140053/4918665)和3.69%(158410/4293580),前后比较,差异有统计学意义(P<0.001)。结论广州市妇女儿童医疗中心优化预约诊疗服务中爽约现象的控制措施有效,值得推广。

Objective In the current situation of normalization prevention and control of COVID-19,we should optimize the control measures in the outpatient appointment service of women's and children's hospitals in order to adapt to the epidemic prevention and control work,and to reduce the waste of medical resources and the cost of hospital management.Methods Based on the hospital information system,the outpatient treatment data of 9212245 cases before the epidemic of COVID-19(from January 1 to December 31,2019)and during the normal prevention and control period of COVID-19(between July 1,2020 and June 30,2021)were obtained.The changes of"break off"and cancelled appointments on the same day before and after the epidemic were compared and analyzed byχ^(2) test.Results The failure rates of appointment services before and after the epidemic were 3.84%(188942/4918665)and 3.78%(162268/4293580),respectively,with a statistical difference(P=0.023).The cancellation rates of appointment on the same day before and after the epidemic were 2.85%(140053/4918665)and 3.69%(158410/4293580),with a statistical difference(P<0.001).Conclusion The control measures of the medical center to optimize the missed appointments in appointment diagnosis and treatment service are effective and worthy of promotion.
