腹腔镜子宫切除术患者围术期卵巢血流、储备及局部微循环的变化研究

Study on Changes of Ovarian Blood Flow, Reserve and Local Microcirculation in Patients Undergoing Laparoscopic Hysterectomy during Perioperative Period

目的:探究及观察腹腔镜子宫切除术患者围术期卵巢血流、储备及局部微循环的变化情况。方法:选取2019年6月-2021年6月120例子宫切除术患者作为研究对象,按照随机数字表法将患者分为对照组和观察组,每组60例。对照组采用开腹子宫切除术,观察组采用腹腔镜子宫切除术。比较两组术前及术后1、3个月卵巢血流指标及卵巢储备指标(血清AMH、INHB)、术前及术后3、7 d局部微循环指标(局部血管清晰度、红细胞聚集情况)。结果:术前,两组卵巢血流指标、卵巢储备指标及局部微循环指标比较,差异均无统计学意义(P>0.05);术后1、3个月,观察组PI、RI均显著低于对照组,其他卵巢血流指标、卵巢储备指标均显著高于对照组,差异均有统计学意义(P<0.05);观察组术后3、7 d局部微循环指标均显著优于对照组(P<0.05)。结论:腹腔镜子宫切除术对患者的卵巢血流、储备及局部微循环的不良影响相对较小,在本类手术患者中的应用价值相对较高。

Objective:To investigate and observe the changes of ovarian blood flow,reserve and local microcirculation of patients Undergoing laparoscopic hysterectomy during perioperative period.Method:A total of 120 patients with hysterectomy from June 2019 to June 2021 were chosen as the research objects,and they were divided into the control group and the observation group by the method of random number table,60 cases in each group.Open hysterectomy was performed in the control group and laparoscopic hysterectomy was performed in the observation group.Then the ovarian blood flow indexes and ovarian reserve indexes (serum AMH and INHB) before operation and at 1st and 3rd month after operation,local microcirculation indexes (local vascular clarity and erythrocyte aggregation situation) before operation and at 3rd and 7th d after operation of two groups were compared.Result:The ovarian blood flow indexes,ovarian reserve indexes and local microcirculation indexes of two groups before operation were compared,there were no statistically significant differences (P>0.05).At 1st and 3rd month after operation,the PI and RI of the observation group were significantly lower than those of the control group,other ovarian blood flow indexes and ovarian reserve indexes were significantly higher than those of the control group,there were statistically significant differences (P<0.05).At 3rd and 7th d after operation,the local microcirculation indexes the observation group were significantly better than those of the control group,there were statistically significant differences (P<0.05).Conclusion:Laparoscopic hysterectomy has less adverse effects on patients’ ovarian blood flow,reserve and local microcirculation,and its application value in these patients with operation is relatively higher.
