同种异体血管治疗腹主动脉移植物感染的临床应用进展

Progression in clinical application of cryopreserved vascular allografts in the treatment of abdominal aortic graft infection

腹主动脉移植物感染的致残率和致死率均较高,标准的外科治疗方式包括切除感染的移植物、动脉及周围组织,同时恢复受累器官和肢体的血运。解剖外血管旁路和原位重建是主要手术治疗方式,多种因素影响外科手术治疗的预后。迄今为止,临床上有多种血管移植物用于恢复血运重建,主要包括利福平或银涂层的人工血管以及自体静脉、冷冻保存的同种异体血管或牛心包等生物材料,但对于最佳的血管移植物的选择并未达成共识。近年来,临床应用研究证实冻存的同种异体血管用于治疗移植物感染性疾病的结果是可以接受的,但移植物相关并发症不容忽视。本文对冻存的同种异体血管用于治疗腹主动脉移植物感染的临床应用进展进行综述。

Once an abdominal aortic graft infection occurs,it is often catastrophic associated without high morbidity and mortality.Standard surgical treatment includes removing infected grafts,arteries,and surrounding tissues,and revascularization of blood supply to organs and limbs.Extra-anatomical vascular bypass and in-situ reconstruction are the main surgical treatment methods,and many factors affect the prognosis of surgical treatment.So far,there are a variety of vascular grafts used to restore revascularization in clinical practice,mainly including rifampicin or silver-coated artificial blood vessels,autologous veins,cryopreserved allogeneic blood vessels,or bovine pericardium and other biological materials.There is no consensus on the selection of the best vascular graft.In recent years,clinical application studies have confirmed that the results of cryopreserved allogeneic blood vessels for the treatment of graft infectious diseases are acceptable,but the complications related to grafts should not be ignored.This article reviews the clinical application progress of cryopreserved allogeneic blood vessels for the treatment of abdominal aortic graft infection.
