鼻中隔缝合技术中三种缝线布局的比较研究

Comparative study of three different suture layouts in nasal septum penetrating suture technique

目的探讨鼻中隔矫正术中进行连续贯穿缝合法采用不同缝线布局的优缺点。方法选取我院2019年1月~2020年6月共67例因鼻中隔偏曲而行鼻中隔矫正术的患者,在完成中隔矫正术后采用连续贯穿缝合技术避免鼻腔填塞,将所有患者分为三组,A组(24例)采用"口"型缝线布局,B组(21例)采用"弓"形布局,C组(22例)采用"W"形布局。比较各组患者在手术耗时、术后鼻腔出血量、鼻中隔血肿出现率、缝线松散率及线结松脱率方面的差异。结果三种不同缝线布局的手术耗时及术后出血量差异无统计学意义;各组术后鼻中隔血肿出现率差异无统计学意义(P>0.05),而缝线松散率三组间两两比较差异无统计学意义(P>0.017),而A组线结脱落率较B组及C组差异均具有统计学意义(P<0.017),B组和C组之间差异无统计学意义(P>0.05)。结论鼻中隔矫正术进行连续贯穿缝合采用"口"型缝线布局相比采用"弓"型或"W"型缝线布局可有效减少线结脱落率,但三种布局在手术耗时、术后出血量及鼻中隔血肿、缝线松散率上无显著差别。

Objective To explore the advantages and disadvantages of using three different suture layouts in continuously penetrating suture during nasal septum correction.Methods A total of 67 patients who underwent nasal septum correction due to nasal septum deviation from January 2019 to June 2020 in our hospital were selected.After the septum correction was completed,continuously penetrating suture technique was used to avoid nasal cavity packing.All patients were divided into three groups.Group A(24 cases)used the“口”-shaped suture layout,group B(21 cases)used the“弓”-shaped layout,and group C(22 cases)used the“W”-shaped layout.The differences in the operation times,postoperative nasal bleedings,the incidence of nasal septal hematoma,the rate of loose stitches and the rate of loose suture knots in each group were compared.Results The operation times and postoperative bleedings of the three different suture layouts were not statistically significant;there was no significant difference in the incidence of postoperative nasal septal hematoma in each group(P>0.05),and the rate of loose stitches among the three groups There was no statistically significant difference in pairwise comparison(P>0.017),while the rate of loose suture knots in group A was statistically significant compared to group B and group C(P<0.017),and there was no difference statistical significance between group B and group C(P>0.05).Conclusion The continuously penetrating suture of the nasal septum correction surgery using the“口”type suture layout can effectively reduce the rate of loose suture knots compared with the“弓”type or“W”type suture layouts,but there were no significant differences in the surgical times,the amount of postoperative bleedings,the hematoma of the nasal septum,and the rate of loose stitches among the three suture layouts.
