摘要
目的探讨人工听小骨和自体听小骨植入在传导性耳聋患者中的应用效果。方法选取2018年3月至2021年3月我科收治的100例传导性耳聋患者作为研究对象,根据奇偶数顺序将患者随机分为对照组与观察组,每组各50例。其中观察组患者实施人工听小骨植入术治疗,对照组患者实施自体听小骨植入术治疗,分别比较两组患者的术后平均气骨导差、术后平均骨导听力阈值、听力恢复情况以及术后不良反应发生率。结果观察组传导性耳聋患者的术后平均气骨导差[(21.34±5.36)dB]显著优于对照组[(29.05±5.22)dB],两组比较差异具有统计学意义(t=7.287,P<0.001);术后平均骨导听力阈值[(22.28±7.04)dB]明显优于对照组[(28.73±7.19)dB],两组比较差异具有统计学意义(t=4.532,P<0.001);听力恢复情况[74.00%(37/50)]显著优于对照组[26.00%(13/50)],两组比较差异具有统计学意义(统计值=23.04,P<0.001);术后不良反应发生率[2.00%(1/50)]显著低于对照组[14.00%(7/50)],两组比较差异具有统计学意义(统计值=4.891,P=0.027)。结论人工听小骨植入在传导性耳聋患者中应用效果良好,能够明显改善患者的听力水平,具有临床推广价值。
Objective To investigate the effect of artificial ossicles and autogenous ossicles implantation in patients with conductive deafness.Methods 100 cases of conductive deafness admitted to our department from March 2018 to March 2021 were selected as the research objects.According to the order of odd and even numbers,the patients were randomly divided into a control group and an observation group,with 50 cases in each group.Patients in the observation group were treated with artificial ossicles implantation,and patients in the control group were treated with autologous ossicles implantation.The average postoperative air-bone conduction difference,postoperative average bone conduction hearing threshold,hearing recovery and the incidence of postoperative adverse reactions were compared between the two groups.Results The average postoperative air-bone conduction difference[(21.34±5.36) dB]of the observation group of conductive deafness patients was significantly better than that of the control group[(29.05±5.22) dB].The difference between the two groups was statistically significant (t=7.287,P<0.001);the average bone conduction hearing threshold after surgery[(22.28±7.04)dB]was significantly better than the control group[(28.73±7.19)dB],the difference between the two groups was statistically significant (t=4.532,P<0.001);Hearing recovery[74.00% (37/50)]was significantly better than the control group[26.00% (13/50)],the difference between the two groups was statistically significant (statistical value=23.04,P <001);The incidence of postoperative adverse reactions[2.00% (1/50)]was significantly lower than that of the control group[14.00% (7/50)],and the difference between the two groups was statistically significant (statistical value=4.891,P=0.027).Conclusion Artificial hearing ossicle implantation has a good application effect in patients with conductive deafness.It can significantly improve the hearing level of patients and has clinical promotion value.
