椎间孔镜技术治疗腰椎间隙感染的效果

Effect of intervertebral foraminal technique in the treatment of lumbar intervertebral space infection

目的观察椎间孔镜技术治疗腰椎间隙感染的效果。方法选取2017年1月至2020年1月上饶市人民医院收治的56例腰椎间隙感染患者作为研究对象,按照交替分组方法将其分为对照组(28例)与实验组(28例)。对照组采用传统开放手术治疗方法,实验组采用椎间孔镜技术治疗。观察两组患者的手术基本情况、住院时间、住院费用、疼痛度、感染指标、治疗总有效率。结果实验组患者的手术时间、住院时间短于对照组,术中出血量少于对照组,住院费用低于对照组,差异有统计学意义(P<0.05)。术前、术后3 d,两组患者的视觉模拟评分(VAS)评分比较,差异无统计学意义(P>0.05)。术后3 d,两组患者的VAS评分均低于术前,差异有统计学意义(P<0.05)。术前、术后3 d,两组患者的红细胞沉降率(ESR)、C-反应蛋白(CRP)水平比较,差异无统计学意义(P>0.05);两组患者术后3 d的ESR、CRP水平均低于术前,差异有统计学意义(P<0.05)。两组患者的治疗总有效率比较,差异无统计学意义(P>0.05)。结论椎间孔镜技术治疗腰椎间隙感染的效果与开放术式相当,但其可明显缩短患者住院时间,减少医疗费用支出,减轻患者痛苦,具有创伤小、术后恢复快等特点。

Objective To observe the effect of intervertebral foraminal technique in treating lumbar intervertebral space infection.Methods A total of 56 patients with lumbar intervertebral space infection admitted to Shangrao People′s Hospital from January 2017 to January 2020 were selected as the research objects,and they were divided into a control group(28 cases)and an experimental group(28 cases)according to the alternate grouping method.The control group was treated with traditional open surgery,and the experimental group was treated with foraminoscopy.The basic operation situation,length of stay,hospitalization cost,pain,infection index and total effective rate were observed in the two groups.Results The operation time and hospitalization time of patients in the experimental group were shorter than those in the control group,the amount of intraoperative blood loss in the experimental group was less than that in the control group,and the hospitalization cost in the experimental group was lower than that in the control group,with statistical significances(P<0.05).There were no significant differences in visual analog scale(VAS)scores before and 3 d after surgery between the two groups(P>0.05).Three days after surgery,VAS scores of the two groups were lower than those before surgery,the differences were statistically significant(P<0.05).There were no significant differences in erythrocyte sedimentation rate(ESR)and C-reactive protein(CRP)levels between the two groups before and 3 d after surgery(P>0.05).Three days after surgery,ESR and CRP levels in the two groups were lower than those before surgery,the differences were statistically significant(P<0.05).There was no significant difference in the total effective rate between the two groups(P>0.05).Conclusion The efficacy of foraminoscope technique in the treatment of lumbar intervertebral space infections is equivalent to that of open surgery,but it can significantly shorten the patients′hospital stay,reduce medical expenses,and alleviate patients′pain.It has the characteristics of less trauma and quicker recovery after surgery.
