瓣膜置换术联合射频消融对瓣膜性心脏病治疗效果及生活质量的影响

Therapeutic effects of valve replacement combined with radiofrequency ablation on valvular heart disease and its effects on life quality of patients

目的探讨瓣膜置换术联合射频消融对瓣膜性心脏病治疗效果及生活质量的影响。方法选取2017年4月至2021年2月风湿性瓣膜病伴房颤的患者共100例,根据在瓣膜置换术的基础上是否行射频消融将入组患者分为对照组(瓣膜置换术)和观察组(瓣膜置换术联合射频消融),每组50例。对2组患者的出院时和出院6个月后窦性心律的转复率的进行比较,比较2组患者并发症(血栓栓塞、心力衰竭、肾功能不全和年病死率的发生)的发生率及治疗后心功能、LVEF的改善情况以及2组患者治疗前和治疗后6个月的生活质量。结果观察组患者出院时和出院6个月后窦性心律的转复率较对照组明显升高(P<0.05);观察组患者并发症(血栓栓塞、心力衰竭、肾功能不全和年病死率的发生)的发生率较对照组无明显变化(P>0.05);治疗后,观察组患者心功能、LVEF的改善较对照组得到明显改善(P<0.05);治疗前,2组患者生活质量无明显差异(P>0.05),治疗后,观察组患者生活质量较对照组明显改善(P<0.05)。结论瓣膜置换术联合射频消融可提高瓣膜性心脏病患者房颤的转复率同时减少患者并发症的发生,明显改善患者的心功能,提高患者的生活质量,对患者的临床应用具有一定的应用价值。

Objective To investigate the therapeutic effects of valve replacement combined with radiofrequency ablation on valvular heart disease and its effects on life quality of patients.Methods One hundred patients with rheumatic valvular disease complicated by fibrillation atrial who were treated by valve replacement surgery in our hospital from April 2017 to February 2021 were enrolled in the study,who were divided into control group and experimental group,with 50 cases in each group.The patients in control group were treated by valve replacement,however,the patients in experimental group were treated by by valve replacement combined with radiofrequency ablation.The recovery rate of sinus rhythm at discharge and 6 months after discharge,and the incidence of complications(thromboembolism,heart failure,renal insufficiency and annual mortality),the improvement status of cardiac function and LVEF as well as life quality were observed and compared between the two groups.Results The recovery rates of sinus rhythm at discharge and 6 months after discharge in observation group were significantly higher than those in control group(P<0.05),however,there was no significant differences in the incidence rates of complications(thromboembolism,heart failure,renal insufficiency and annual mortality)between the two groups(P>0.05).After treatment,the improvement effects of cardiac function and LVEF as well as life quality of patients in experimental group were superior to those in control group(P<0.05).Conclusion The valvular replacement combined with radiofrequency ablation can improve the recovery rate of atrial fibrillation in patients with valvular heart disease,which can reduce the incidence of complications,significantly improve the cardiac function and life quality of patients,which has certain application value in clinical practice.
