PTEN、TIMP-1及XIAP在老年喉癌癌前病变中的表达及临床意义

Expressions of PTEN,TIMP-1 and XIAP in Precancerous Lesions of Elderly Patients with Laryngeal Carcinoma and Their Clinical Significances

目的探讨第10同源染色体丢失性磷酸酶-张力蛋白基因(PTEN)、基质金属蛋白酶抑制剂-1(TIMP-1)及X-连锁凋亡抑制蛋白(XIAP)在老年人喉癌癌前病变中的表达及临床意义。方法收集2018年4月—2020年4月接受手术治疗的97例喉癌新鲜手术切除癌组织,同时选取上述患者距肿瘤3 cm的癌旁正常组织。比较不同组织、生存与死亡患者PTEN、TIMP-1及XIAP表达情况,分析PTEN、TIMP-1及XIAP联合检测对老年人喉癌癌前病变的预测效能。结果喉癌组织中PTEN、TIMP-1阴性表达率高于癌旁组织,XIAP阳性表达率高于癌旁组织(P<0.05)。97例喉癌随访期间病死率为29.90%。死亡组PTEN、TIMP-1阴性表达率高于生存组,XIAP阳性表达率高于生存组(P<0.05)。淋巴结转移、浆膜浸润、分化程度、PTEN阴性、TIMP-1阴性、XIAP阳性为喉癌患者死亡的独立危险因素(P<0.05,P<0.01)。PTEN、TIMP-1、XIAP三者联合检测敏感度、特异度高于PTEN、TIMP-1、XIAP单一检测(P<0.05)。结论PTEN、TIMP-1及XIAP联合检测有助于老年人喉癌癌前病变的早期诊断。

Objective To investigate expressions and clinical significances of phosphatase and tensin homolog deleted on chromosome 10(PTEN),matrix metallo-proteinase inhibitor-1(TIMP-1)and X-linked inhibitor of apoptosis protein(XIAP)in precancerous lesions of elderly patients with laryngeal carcinoma.Methods The fresh surgically resected cancer tissues of 97 patients with laryngeal cancer who had undergone surgeries between April 2018 and April 2020 were collected,and the adjacent normal tissues 3 cm away from the tumor were also selected.Expressions of PTEN,TIMP-1 and XIAP were compared in different tissues,surviving and dead patients,and the efficacy of combined detection of PTEN,TIMP-1 and XIAP in prediction of precancerous lesions of elderly patients with laryngeal cancer was analyzed.Results The negative expression rates of PTEN and TIMP-1 and the positive expression rate of XIAP in laryngeal carcinoma tissues were significantly higher than those in adjacent tissues(P<0.05).The mortality of 97 patients with laryngeal carcinoma during the follow-up was 29.90%.The negative expression rates of PTEN and TIMP-1 and the positive expression rate of XIAP in death group were significantly higher than those in survival group(P<0.05).Lymph node metastasis,serous infiltration,degree of differentiation,negative PTEN,negative TIMP-1 and positive XIAP were independent risk factors for death in patients with laryngeal carcinoma(P<0.05,P<0.01).The sensitivity and specificity of combined detection of PTEN,TIMP-1 and XIAP were significantly higher than those by detections of PTEN,TIMP-1 and XIAP alone(P<0.05).Conclusion Combined detection of PTEN,TIMP-1 and XIAP is helpful in the early diagnosis of precancerous lesions of elderly patients with laryngeal cancer.
