MRI与16层螺旋CT扫描诊断乙型肝炎肝硬化背景的小肝癌患者的临床价值分析

Clinical value of magnetic resonance imaging and 16 slice spiral CT in diagnosis of small hepatocellular carcinoma with hepatitis B cirrhosis background

目的分析MRI与16层螺旋CT扫描诊断乙型肝炎(以下简称乙肝)肝硬化背景小肝癌的临床价值。方法选取2017年3月至2020年3月濮阳市油田总医院收治的106例乙肝肝硬化背景的疑似小肝癌患者作为研究对象,采用随机数字表法分为A组和B组,每组53例。A组采用16层螺旋CT扫描检查,B组采用MRI检查,将两组诊断结果与手术或穿刺活检病理诊断结果进行对比分析。结果手术病理诊断结果显示,A组与B组阳性检出率分别为75.47%、77.36%(P>0.05)。A组敏感性、特异性、诊断准确率分别为80.00%、76.92%、79.25%,B组敏感性、特异性、诊断准确率分别为95.12%、91.67%、94.34%,两组敏感性、特异性、诊断准确率比较,差异有统计学意义(P<0.05)。结论MRI诊断乙肝肝硬化背景的小肝癌具有较高的敏感性和特异性,且诊断准确率高,其诊断效果优于16层螺旋CT扫描检查。

【Objective】To analyze the clinical value of magnetic resonance imaging technology and 16-slice spiral CT scan in the diagnosis of small liver cancer with hepatitis B liver cirrhosis.【Methods】A total of 106 patients with hepatitis B liver cirrhosis and suspected small liver cancer were divided into two groups,A and B,and were examined by 16-slice spiral CT scan and magnetic resonance imaging technology.The diagnosis results of the two groups were compared with the surgical pathological diagnosis results.【Results】The results of surgical pathological diagnosis showed that the positive detection rates of group A and group B were 75.47%and 77.36%,respectively,and there was no significant difference between the groups(P>0.05).The sensitivity,specificity,and diagnostic accuracy of group A were 80.00%,76.92%,79.25%,respectively,the sensitivity,specificity,and diagnostic accuracy of group B were 95.12%,91.67%,and 94.34%,respectively.The sensitivity,specificity and diagnostic accuracy of the two groups were significantly different(P<0.05).【Conclusion】Magnetic resonance imaging technology for the diagnosis of small liver cancer with hepatitis B liver cirrhosis has high sensitivity and specificity,high diagnostic accuracy,and has better diagnostic effect than 16-slice spiral CT scan.
