摘要
目的检测急性白血病患者的免疫指标、凝血指标、基质衍生因子-1α(SDF-1α)和乳酸脱氢酶(LDH)水平,并探讨其临床意义。方法回顾性分析2017年2月至2021年2月在陕西中医药大学附属医院接受治疗的111例急性白血病患者的诊疗资料,其中初诊组51例,完全缓解组60例,选择同期健康体检者78例作为对照组。比较三组受检者的免疫指标[T淋巴细胞4(CD4^(+))、T淋巴细胞8(CD8^(+))、T辅助细胞(CD4^(+)/CD8^(+))]和自然杀伤细胞(NK)水平,以及血小板计数(PLT)、血小板体积(MPV)和SDF-la、LDH水平。结果初诊组和完全缓解组患者的CD4^(+)、CD4^(+)/CD8^(+)和NK水平明显低于对照组,CD8^(+)水平明显高于对照组,而初诊组患者的CD4^(+)、CD4^(+)/CD8^(+)和NK水平明显低于完全缓解组,CD8^(+)水平明显高于完全缓解组,差异均有统计学意义(P<0.05);初诊组和完全缓解组患者的PLT、MPV水平明显低于对照组,D二聚体水平明显高于对照组,而初诊组患者的PLT、MPV水平明显低于完全缓解组,D二聚体水平明显高于完全缓解组,差异均具有统计学意义(P<0.05);初诊组和完全缓解组患者的SDF-1α、LDH水平明显高于对照组,而初诊组患者的SDF-1α及LDH水平又明显高于完全缓解组,差异均具有统计学意义(P<0.05)。结论急性白血病患者的免疫指标、凝血指标、SDF-1α、LDH的表达均异常于正常人,其水平变化可作为评估病情的依据。
Objective To detect the levels of immune indexes,coagulation indexes,stromal cell-derived factor-1α(SDF-lα)and lactate dehydrogenase(LDH)in patients with acute leukemia,and to explore its clinical significance.Methods The diagnosis and treatment data of 111 patients with acute leukemia treated in the Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine from February 2017 to February 2021 were analyzed retrospectively.Of them,there were 51 patients in the newly diagnosed group,and 60 patients in complete remission group.At the same time,78 healthy subjects in the same period were selected as the control group.The levels of immune indexes[T lymphocyte 4(CD4^(+)),T lymphocyte 8(CD8^(+)),T helper cells(CD4^(+))/CD8^(+)],natural killer cells(NK),platelet count(PLT),platelet volume(MPV),SDF-lα,and LDH were compared among the three groups.Results The levels of CD4^(+),CD4^(+)/CD8^(+)and NK in the newly diagnosed group and complete remission group were significantly lower than those in the control group(P<0.05);the level of CD8^(+)was significantly higher than that in the control group(P<0.05);and the levels of CD4^(+),CD4^(+)/CD8^(+),and NK in the newly diagnosed group were significantly lower than those in the complete remission group,and the level of CD8^(+)was significantly higher than that in complete remission group,with statistically significant differences(P<0.05).The levels of PLT and MPV in the newly diagnosed group and complete remission group were significantly lower than those in the control group,and the levels of D dimer in the newly diagnosed group and complete remission group were significantly higher than those in the control group;the levels of PLT and MPV in the newly diagnosed group were significantly lower than those in the complete remission group,and the level of D dimer was significantly higher than that in complete remission group,with statistically significant differences(P<0.05).The levels of SDF-1α,LDH in newly diagnosed group and complete remission group were significantly higher than those in the control group,and the levels of SDF-1αand LDH in newly diagnosed group were significantly higher than those in complete remission group,with statistically significant differences(P<0.05).Conclusion The expression of immune indexes,coagulation indexes,matrix derived factor la(SDF-1α)and lactate dehydrogenase(LDH)in patients with acute leukemia are all abnormal than normal people,and their changes can be used as the basis for the evaluation of the disease.
