活血通络清脑汤治疗短暂性脑缺血发作性眩晕的临床研究

Clinical study of Huoxue Tongluo Qingnao Decoction in treating vertigo due to transient ischemic attack

目的观察活血通络清脑汤治疗短暂性脑缺血发作性眩晕的临床效果。方法回顾性分析榆林市中医医院脑病科2017年10月至2020年10月收治的204例短暂性脑缺血患者的临床诊治资料,依据治疗方式不同分为对照组(n=101,常规治疗)和研究组(n=103,常规治疗+活血通络清脑汤治疗),疗程均为1个月。比较两组患者治疗后的临床疗效、治疗前后的血脂水平[高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)及总胆固醇(TC)]、血液流变学指标[全血黏度、凝血酶原时间、纤维蛋白原、红细胞压积]、眩晕症状及不良反应发生情况。结果研究组患者的临床治疗总有效率为97.09%,明显高于对照组的89.11%,差异具有统计学意义(P<0.05);治疗后,两组患者的TC、TG、LDL-C较治疗前均下降,HDL-C较治疗前均升高,且以研究组变化最为明显,差异均具有统计学意义(P<0.05);治疗后,两组患者的全血黏度、纤维蛋白原、红细胞压积较治疗前下降,凝血酶原时间较治疗前延长,且以研究组患者的变化幅度最为明显,差异均具有统计学意义(P<0.05);治疗后,研究组患者的眩晕症状评分为(0.84±0.27)分,明显低于对照组的(0.96±0.28)分,差异具有统计学意义(P<0.05);研究组和对照组患者治疗期间的不良反应发生率分别为4.85%、6.93%,差异无统计学意义(P>0.05)。结论在常规治疗基础上采用活血通络清脑汤治疗短暂性脑缺血发作性眩晕可减轻患者的眩晕症状,改善血液流变学及血脂水平,且安全性高,具有临床应用价值。

Objective To observe the clinical effect of Huoxue Tongluo Qingnao Decoction in the treatment of transient ischemic attack vertigo.Methods The clinical diagnosis and treatment data of 204 patients with transient ischemic admitted to Yulin Hospital of Traditional Chinese Medicine from October 2017 to October 2020 were retrospectively analyzed.According to different treatment methods,they were divided into study group(n=103,routine treatment)and control group(n=101,routine treatment+Huoxue Tongluo Qingnao Decoction),both for 1 month.The clinical efficacy,blood lipid levels[high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),total cholesterol(TC),triglyceride(TG)],hemorheology indexes(whole blood viscosity,prothrombin time,fibrinogen,hematocrit),vertigo symptoms,and adverse reactions were compared between the two groups.Results The total effective rate in the study group was 97.09%,which was significantly higher than 89.11%of the control group(P<0.05).After treatment,TC,TG,LDL-C in the two groups decreased compared with those before treatment,and HDL-C increased compared with those before treatment;the changes in the study group were more significant than those in the control group(P<0.05).After treatment,the whole blood viscosity,fibrinogen,and hematocrit of parents of the two groups were lower than those before treatment,and the prothrombin time was longer than those before treatment;the change in the study group was more significant than those in the control group(P<0.05).After treatment,the dizziness score of the study group was(0.84±0.27)points,which was significantly lower than(0.96±0.28)points of the control group(P<0.05).The incidence of adverse reactions during the treatment of the study group and the control group were 4.85%,6.93%,respectively,and the differences were statistically significant(P>0.05).Conclusion Huoxue Tongluo Qingnao Decoction in the treatment with transient ischemic attack vertigo can reduce the dizziness of patientson the basis of conventional treatment,improve the patient’s hemorheology and blood lipid levels,and has high safety and clinical application value.
