CT平扫检查对急性阑尾炎的诊断价值探讨

The value of CT plain scan in the diagnosis of acute appendicitis

目的 探究CT平扫检查对急性阑尾炎的诊断价值。方法 500例疑似急性阑尾炎患者,均进行CT平扫检查,并以手术病理作为“金标准”,观察CT平扫检查在急性阑尾炎中的诊断效能和具体征像。结果 手术病理检出急性单纯性阑尾炎52例,急性化脓性阑尾炎303例,急性坏疽穿孔性阑尾炎50例,检出率为81.00%;CT平扫检出急性单纯性阑尾炎52例,急性化脓性阑尾炎301例,急性坏疽穿孔性阑尾炎49例,检出率为80.40%。CT平扫检查对急性阑尾炎的诊断符合率为98.60%(493/500),敏感性为98.77%(400/405),特异性为97.89%(93/95)。急性阑尾炎的CT征像主要包括阑尾肿大增粗、管径≥6 mm、直径≥2 mm、阑尾腔积液、阑尾腔积气、阑尾粪石、阑尾周围炎和阑尾区域脓肿。结论 对疑似急性阑尾炎患者采用CT平扫检查,能有效明确急性阑尾炎类型,具有较高的诊断符合率、敏感性和特异性。

Objective To investigate the value of CT plain scan in the diagnosis of acute appendicitis.Methods In 500 patients with suspected acute appendicitis,CT scan was performed,and surgical pathology was used as the"gold standard"to observe the diagnostic efficacy and specific signs of CT scan in acute appendicitis.Results Surgical pathology detected 52 cases of acute simple appendicitis,303 cases of acute suppurative appendicitis,and 50 cases of acute gangrenous perforated appendicitis,with a detection rate of 81.00%;CT plain scan detected 52 cases of acute simple appendicitis,301 cases of acute suppurative appendicitis,and 49 cases of acute gangrenous perforated appendicitis,with a detection rate of 80.40%.The coincidence rate of CT plain scan for the diagnosis of acute appendicitis was 98.60%(493/500),the sensitivity was 98.77%(400/405),and the specificity was 97.89%(93/95).The CT signs of acute appendicitis mainly include enlarged and thick appendix,tube diameter≥6 mm,diameter≥2 mm,effusion in the appendix cavity,gas accumulation in the appendix cavity,appendectomy bezoar,periappendicitis and abscess in the appendix area.Conclusion Plain CT scan in the diagnosis of suspected acute appendicitis can effectively identify the type of acute appendicitis,and has a high diagnostic coincidence rate,sensitivity and specificity.
