经皮微创钢板内固定术与切开复位内固定术治疗胫骨干骨折的效果分析 被引量:1

Analysis of the effect of minimally invasive percutaneous plate internal fixation and open reduction internal fixation in the treatment of tibial shaft fracture

摘要 目的 分析经皮微创钢板内固定术与切开复位内固定术治疗胫骨干骨折的效果。方法 45例胫骨干骨折患者,随机分为对照组(23例)和观察组(22例)。对照组患者给予切开复位内固定术治疗,观察组给予经皮微创钢板内固定术治疗。比较两组手术时间、术后疼痛持续时间、使用抗生素时间、骨折愈合时间、住院时间、并发症发生率及治疗前后Lysholm膝关节评分和视觉模拟评分法评分。结果 观察组手术时间(72.21±1.45)min、术后疼痛持续时间(3.01±0.13)d、使用抗生素时间(3.19±0.51)d、骨折愈合时间(15.21±2.67)周、住院时间(9.19±2.12)d均短于对照组的(89.52±4.91)min、(4.56±1.21)d、(5.12±3.14)d、(18.56±4.91)周、(12.21±5.18)d,差异有统计学意义(P<0.05)。治疗后,观察组Lysholm膝关节评分高于对照组,视觉模拟评分法评分低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率4.55%(1/22)低于对照组的26.09%(6/23),差异有统计学意义(P<0.05)。结论 经皮微创钢板内固定术治疗胫骨干骨折,可缩短治疗时间,加速骨折愈合,减少抗生素应用,降低并发症发生率,减轻患者痛苦和改善膝关节功能,值得临床推广。 Objective To analyze the effect of minimally invasive percutaneous plate internal fixation and open reduction internal fixation in the treatment of tibial shaft fracture.Methods A total of 45 patients with tibial shaft fracture were randomly divided into control group(23 cases)and observation group(22 cases).The control group was treated with open reduction internal fixation,and the observation group was treated with minimally invasive percutaneous plate internal fixation.Both groups were compared in terms of operation time,postoperative pain duration,antibiotic use time,fracture healing time,hospitalization time,complication rate,Lysholm knee joint score and visual analogue scale score before and after treatment.Results The operation time(72.21±1.45)min,postoperative pain duration(3.01±0.13)d,antibiotic use time(3.19±0.51)d,fracture healing time(15.21±2.67)weeks,and hospitalization time(9.19±2.12)d of the observation group were shorter than(89.52±4.91)min,(4.56±1.21)d,(5.12±3.14)d,(18.56±4.91)weeks,and(12.21±5.18)d of the control group,and the difference was statistically significant(P<0.05).After treatment,the Lysholm knee joint score of the observation group was higher than that of the control group,and the visual analogue scale score was lower than that of the control group.All the differences were statistically significant(P<0.05).The complication rate of the observation group was 4.55%(1/22),which was lower than 26.09%(6/23)of the control group,and the difference was statistically significant(P<0.05).Conclusion Minimally invasive percutaneous plate internal fixation for the treatment of tibial shaft fracture can shorten the treatment time,accelerate fracture healing,reduce the application of antibiotics,complication rates and patient pain and improve knee joint function,which is worthy of clinical promotion.
