米索前列醇在妇女绝经后取环术中的应用观察

Observation of misoprostol in postmenopausal women undergoing IUD removal

目的 分析米索前列醇在妇女绝经后取环术中的应用。方法 200例实施绝经后取环术妇女,以随机数字表法分为观察组与对照组,各100例。对照组实施常规取环术,观察组实施取环术前阴道内放置米索前列醇治疗。比较两组宫颈软化程度、取环结果、术中疼痛程度。结果 观察组中4号扩宫器能顺利通过宫颈88例(88.00%),不能顺利通过宫颈12例(12.00%);对照组中4号扩宫器能顺利通过宫颈32例(32.00%),不能顺利通过宫颈68例(68.00%)。观察组4号扩宫器能顺利通过宫颈占比高于对照组,差异有统计学意义(χ^(2)=65.333, P=0.000<0.05)。观察组取环顺利占比高于对照组,取环困难及取环失败占比低于对照组,差异有统计学意义(P<0.05)。观察组术中无痛占比高于对照组,轻度疼痛、中度疼痛及重度疼痛占比低于对照组,差异均有统计学意义(P<0.05)。结论 米索前列醇应用在妇女绝经后取环术治疗中,可显著促进宫颈软化,降低妇女术中疼痛,保证手术顺利进行,提升取环效果。

Objective To analyze the application of misoprostol in postmenopausal women undergoing intrauterine contraceptive device(IUD)removal.Methods A total of 200 cases of postmenopausal women undergoing IUD removal were divided into the observation group and the control group by random numerical table,with 100 cases in each group.The control group received conventional IUD removal,and the observation group received misoprostol before IUD removal.The degree of cervical softening,results of IUD removal,and degree of intraoperative pain were compared between the two groups.Results In the observation group,the No.4 uterine dilator was able to pass the cervix in 88 cases(88.00%)and could not pass the cervix in 12 cases(12.00%);in the control group,the No.4 uterine dilator was able to pass the cervix in 32 cases(32.00%)and could not pass the cervix in 68 cases(68.00%).The proportion of No.4 uterine dilator in the observation group that successfully passed through the cervix was higher than that in the control group,and the difference was statistically significant(χ^(2)=65.333,P=0.000<0.05).The percentage of successful IUD removal in the observation group was higher than that in the control group,and the percentage of difficult IUD removal and failed IUD removal was lower than that in the control group.All the differences were statistically significant(P<0.05).The percentage of intraoperative pain-free in the observation group was higher than that in the control group,and the percentages of mild pain,moderate pain and severe pain was lower than those in the control group.All the differences were statistically significant(P<0.05).Conclusion Application of misoprostol in postmenopausal women undergoing IUD removal can significantly promote cervical softening,reduce the intraoperative pain,ensure the smooth operation and improve the effect of IUD removal.
