基于数据挖掘的黄力教授治疗稳定性冠心病用药规律分析

Analysis on Prescription Rule of Professor Huang Li in Treating Stable Coronary Artery Disease Based on Data Mining

目的基于数据挖掘方法分析黄力教授治疗稳定性冠心病的用药规律。方法收集黄力教授2013年1月—2018年12月在中日友好医院中西医结合心内科门诊部的治疗显效及有效的稳定性冠心病医案,利用Microsoft Office Excel 2013软件建立方药数据库,采用SPSS Statistics 20.0和SPSS Modeler 18.0软件对数据进行频数分析、关联规则分析、复杂网络图分析和聚类分析。结果纳入医案156份,处方156首,中药206味。高频药物包括黄芪、葛根、川芎、酸枣仁、三七、红景天、山药、生地黄、茯苓、白术等30味,占比57.48%,功效可归为补气药、活血化瘀药、安神药等11类,药味多甘、辛、苦,药性多平、温,多归脾、肝、肺、心经。关联规则分析得到24组关联规则,置信度≥80%的规则有17组。高频药物聚类分析得到6个聚类方。结论黄力教授临床治疗稳定性冠心病重视气血,以气虚血瘀、心脉痹阻为基本病机,治疗多从脾、肝、肺、心出发,以益气活血、化瘀止痛立法,或佐以安神定志、行气利水、化痰祛浊、平肝理气及清热解表之品,同时注重经方的加减运用。

Objective To analyze the prescription rule of professor Huang Li in treating stable coronary artery disease by data mining methods.Methods Clinical records of coronary artery disease were collected.Recipe database was established by Microsoft Office Excel 2013.SPSS Statistics 20.0 and SPSS Modeler 18.0 were used for frequency analysis,association rule analysis,complex network diagram analysis,and cluster analysis.Results One hundred and fifty-six medical cases,156 prescriptions,and 206 traditional Chinese medicines were included.A total of 30 high frequency drugs were selected,which mainly included Astragalus Membranaceus,Pueraria Lobata,Ligusticum Chuanxiong,Ziziphus Jujuba,Panax Notoginseng,Rhodiola Crenulata,Dioscorea Opposita,Rehmannia Glutinosa,Poria Cocos,and Atractylodes Macrocephala,accounting for 57.48%.The efficacy was classified into 11 categories,such as invigorating qi medicine,promoting blood circulation and removing stasis medicine,and tranquilizing medicine.The flavor belonged to sweet,pungent or bitter.The property belonged to mild or warm mostly.The main channels related to spleen,liver,lung or heart.Twenty-four groups of association rules were obtained from association rule analysis,and 17 groups of rules with confidence≥80%were obtained.High frequency drug cluster analysis obtained 6 clusters.Conclusion Professor Huang Li paid more attention to qi and blood in the clinical treatment of stable coronary artery disease,qi deficiency and blood stasis,heart block were the basic pathogenesis and the treatment were mostly based on spleen,liver,lung and heart.
