温阳通络法对功能性肛门直肠痛抗炎作用及肌肉痉挛痛影响

Effects of Wenyang Tongluo Method on anti-inflammatory effect and muscle spasm pain of functional anorectal pain

目的:观察温阳通络法治疗功能性肛门直肠痛的临床疗效,分析其抗炎效果和对肌肉痉挛痛改善的影响。方法:选择2019年8月—2021年1月收治的108例功能性肛门直肠痛患者作为研究对象,通过盲选法随机分为2组,各54例。对照组实施生物反馈疗法,研究组在此基础上联合温阳通络法治疗,连续治疗3个疗程。治疗前后,检测2组患者血清炎性因子肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、C反应蛋白(CRP)水平变化情况;采用视觉模拟评分(VAS)评价肛门肌肉痉挛痛缓解程度;记录2组患者肛门直肠测压结果,对2组的疗效情况进行分析比较。结果:研究组临床疗效优于对照组(88.89%vs 66.67%,P<0.05)。治疗后,研究组血清TNF-α、IL-6、CRP水平均低于对照组(P<0.05);研究组肛管静息压、肛管最大收缩压及持续挤压时间及VAS评分均低于对照组(P<0.05)。结论:温阳通络法辅助治疗功能性肛门直肠痛的疗效优于单独采用生物反馈疗法,能够减轻炎症反应,缓解肌肉痉挛痛。

Objective: To observe the clinical efficacy of Wenyang Tongluo Method in the treatment of functional anorectal pain, and to analyze its anti-inflammatory effect and its effect on the improvement of muscle spasm pain. Methods: A total of 108 patients with functional anorectal pain admitted to the department of Anorectal of our hospital from August 2019 to January 2021 were selected as the research objects and randomly divided into two groups by blind selection method, with 54 patients in each group. The control group was treated with biofeedback therapy, and the research group was treated with hot compress for 3 courses. Before and after treatment, the serum inflammatory factor and tumor necrosis factor were detected-α(TNF-α) 、Changes of interleukin-6(IL-6) and C-reactive protein(CRP);visual analogue scale(VAS) was used to evaluate the pain relief of anal muscle spasm. The anorectal manometry results of the two groups were recorded, and the curative effects of the two groups were analyzed and compared. Results: The clinical efficacy of the study group was better than that of the control group(88.89% vs 66.67%, P<0.05). After treatment, the serum levels of tumor necrosis factor-α(TNF-α), interleukin-6(IL-6) and C-reactive protein(CRP) in the study group were lower than those in the control group(P<0.05). The anal resting pressure, maximum anal systolic pressure, continuous extrusion time and VAS score in the study group were lower than those in the control group(P<0.05). Conclusion: The effect of Wenyang Tongluo Method in the adjuvant treatment of functional anorectal pain is better than biofeedback therapy alone. It can reduce inflammatory reaction and muscle spasm pain.
