粪便脱落细胞IKZF1甲基化联合血清CEA、CA50检测对结肠癌的诊断价值分析 被引量:3

Diagnostic value of IKZF1 methylation in fecal exfoliated cells combined with serum CEA and CA50 detection in cancer of colon

摘要 目的:通过检测粪便脱落细胞Ikaros家族锌指蛋白1(IKZF1)甲基化、血清CEA和CA50在结肠癌患者中的表达情况,探讨三者对结肠癌的诊断价值。方法:选取2014年4月—2019年6月已确诊的结肠癌110例患者作为观察组,60例健康体检者作为对照组。采用PCR方法检测粪便脱落细胞IKZF1甲基化状况;全自动酶标仪检测CEA、CA50表达,采用四格表对IKZF1甲基化联合血清CEA、CA50对结肠癌的诊断价值进行评估。结果:与对照组比较,观察组的粪便脱落细胞IKZF1甲基化、血清CEA、CA50和三者联合检测的阳性率均显著增加(P<0.05)。结肠癌患者粪便脱落细胞IKZF1甲基化、血清CEA、CA50及三者联合检测阳性率与年龄、性别无相关性(P>0.05),而与Dukes分期、分化程度和淋巴结转移有相关性(P<0.05)。粪便脱落细胞IKZF1甲基化、血清CEA、CA50三者联合对结肠癌进行检测时,其灵敏度为72.73%、特异度为90.00%、准确度为78.82%、约登指数为62.73%。结论:粪便脱落细胞IKZF1甲基化联合血清CEA、CA50对结肠癌的诊断价值较高,有望应用临床诊断中。 Objective: To explore the expression of IKZF1(Ikaros family zinc finger 1) methylation in stool exfoliated cells, serum CEA and CA50 in patients with cancer of colon, and to explore their diagnostic value for cancer of colon. Methods: A total of 110 patients with cancer of colon diagnosed and treated in our hospital from April 2014 to June 2019 were selected as the observation group and 60 healthy people as the control group. The methylation status of IKZF1 in fecal exfoliated cells was detected by PCR;the expression of CEA and CA50 was detected by automatic enzyme labeling instrument, and the diagnostic value of IKZF1 methylation combined with serum CEA and CA50 in cancer of colon was evaluated by four grid table. Results: Compared with those in the control group, the positive rates of IKZF1 methylation in fecal exfoliated cells, serum CEA, CA50 and combined detection were significantly higher(P<0.05). The positive rates of IKZF1 methylation in stool exfoliated cells, serum CEA, CA50 and combined detection of the three is not related to age and gender(P>0.05), but is related to Dukes staging, degree of differentiation and lymph node metastasis(P<0.05). The sensitivity, specificity, accuracy and Youden index of IKZF1 methylation in stool exfoliated cells, serum CEA, CA50 and combined detection in detecting cancer of colon were 72.73%, 90.00%, 78.82% and 62.73%, respectively. Conclusion: IKZF1 methylation of faecal exfoliated cells combined with serum CEA and CA50 has high diagnostic value for cancer of colon, and it is expected to be applied in clinical diagnosis.
