摘要
目的观察阿奇霉素联合布地奈德雾化吸入治疗儿童支原体肺炎的临床效果。方法选取2018年1月-2020年1月佛山市南海区妇幼保健院收治的儿童支原体肺炎患儿52例为研究对象,根据随机数字表法分为观察组和对照组,每组26例。对照组予以阿奇霉素治疗,观察组在对照组基础上联合布地奈德雾化吸入治疗。比较2组患儿症状改善时间,治疗总有效率,治疗前后炎性因子水平及肺功能指标。结果观察组发热消退、咳嗽停止、肺部啰音消失时间及住院时间均短于对照组(P<0.01);观察组治疗总有效率为96.15%,高于对照组的73.08%(χ^(2)=3.842,P=0.050);治疗后,2组干扰素-γ(IFN-γ)水平较治疗前升高、白细胞介素-4(IL-4)水平较治疗前下降,且观察组IFN-γ水平高于对照组、IL-4水平低于对照组(P<0.05或P<0.01);2组第1秒用力呼气容积(FEV_(1))、呼气峰值流速(PEF)和用力肺活量(FVC)均较治疗前改善,且观察组各项指标水平高于对照组(P<0.05或P<0.01)。结论儿童支原体肺炎应用阿奇霉素联合布地奈德雾化吸入治疗可增强临床效果,有助于控制支原体感染,消除气道炎性反应,缓解患儿咳嗽、发热等临床症状,促进患儿快速康复。
Objective To observe the clinical efficacy of azithromycin combined with budesonide nebulized inhalation in the treatment of mycoplasma pneumonia in children. Methods From January 2018 to January 2020,52 children with mycoplasma pneumonia admitted to the Maternal and Child Health Hospital of Nanhai District in Foshan were selected as the research objects. According to the random number table method,they were divided into observation group and control group,with 26 cases in each group. The control group was treated with azithromycin,and the observation group was combined with budesonide nebulized inhalation on the basis of the control group. Compared the time of symptom improvement of the two groups of children,and the total effective rate of treatment. The levels of inflammatory factors and lung function indexes before and after treatment were compared between the two groups. Results The observation group’ s fever subsided,coughing stopped,lung rales disappearing time and hospital stay were shorter than those in the control group( P < 0. 01). The total effective rate of treatment in the observation group was 96. 15%,which was higher than 73. 08% in the control group( χ;=3. 842,P = 0. 050). After treatment,the levels of IFN-γ in the two groups were higher than those before treatment,and the levels of IL-4 were lower than those before treatment,IFN-γ levels in the observation group were higher than those in the control group and IL-4 level was lower than the control group( P < 0. 05 or P < 0. 01);The FEV_(1),PEF and FVC in the two groups were improved compared with those before treatment,and the levels of various indicators in the observation group were higher than those in the control group( P < 0. 05 or P < 0. 01). Conclusion The application of azithromycin combined with budesonide nebulized inhalation in the treatment of mycoplasma pneumonia in children can enhance the clinical effect,help control mycoplasma infection,eliminate airway inflammatory reactions,alleviate clinical symptoms such as cough and fever in children,and promote rapid recovery of children.
