无细胞脂肪组织提取液在皮肤及软组织再生修复中的应用

Cell-free fat extract for the regeneration and repair of skin and soft tissue defects

创伤、感染、肿瘤等各种原因导致的大面积皮肤及软组织缺损是整形外科的常见病,皮肤扩张、皮瓣移植、脂肪组织填充等是组织缺损修复的常用技术手段,而这些技术在应用过程中仍然存在组织扩张速度缓慢、扩张皮肤回缩、移植皮瓣坏死、填充脂肪吸收率高等难题。我们前期建立了从人体脂肪组织中提取无细胞脂肪组织提取液(Cell-free fat extract,CEFFE)技术,该技术可以获得富含各类生长因子而不含活细胞的液体,具有调节炎症、促血管新生、促细胞增殖、抗细胞凋亡、抑制活性氧产生等多种生物学功能。动物实验证实,CEFFE在皮瓣移植、皮肤扩张、脂肪移植、创面愈合、皮肤光老化等疾病中均有治疗效果。本文就CEFFE技术在皮肤及软组织再生修复中的应用及前景进行综述。

Large skin-soft tissue defects caused by trauma,infection or tumor are common problems in plastic and reconstructive surgery.The most commonly used method to repair such defects include skin expansion,skin grafting as well as fat grating etc.However,there are shortcomings in these techniques such as long amplification cycle period and retraction of the expanded skin,flap necrosis and high absorption rate of grafted fats.We previously developed a Cell-free fat extract(CEFFE)preparation technique from human adipose tissue.This technique allows to obtain a cell-free liquid enriched with various growth factors and possess multiple biological functions such as inflammatory regulation,pro-angiogenic,pro-proliferative,anti-apoptotic and antioxidant effects.Animal studies have proved therapeutic effects of CEFFE on skin flap survival,skin expansion,fat graft survival,wound healing and photodamaged skin treatment.In this paper,current knowledge and future prospects of CEFFE technique in the regeneration and repair of skin and soft tissue defects were reviewed.
