半夏泻心汤加减对脾胃湿热型慢性萎缩性胃炎患者免疫功能与炎性因子水平的影响

Effects of Banxia Xiexin decoction on immune function and inflammatory cytokines levels in patients with chronic atrophic gastritis of spleen-stomach damp-heat type

目的分析半夏泻心汤加减对脾胃湿热型慢性萎缩性胃炎患者临床疗效、免疫功能、血清白细胞介素-2(IL-2)、白细胞介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)水平的影响。方法选取四川省第二中医医院于2017年6月至2021年2月收治的160例脾胃湿热型慢性萎缩性胃炎患者作为研究对象,采用随机数字表法分为对照组和研究组,各80例。给予对照组患者常规药物治疗(兰索拉唑片、阿莫西林胶囊及克拉霉素胶囊),研究组患者在对照组的基础上使用半夏泻心汤加减辨治,两组患者均连续治疗1个月。比较两组患者临床疗效、主要中医证候积分,CD3^(+)、CD4^(+)、CD8^(+)百分比和CD4^(+)/CD8^(+)比值,以及血清IL-2、IL-6、TNF-α水平。结果治疗后,观察组患者治疗总有效率为93.75%,高于对照组的73.75%;两组患者CD3^(+)、CD4^(+)百分比及CD4^(+)/CD8^(+)比值较治疗前均升高,且研究组高于对照组;两组患者中医证候积分、CD8^(+)百分比、血清IL-2、IL-6、TNF-α水平较治疗前均降低,且研究组低于对照组(均P<0.05)。结论联合应用半夏泻心汤加减治疗能显著提高脾胃湿热型慢性萎缩性胃炎患者的临床疗效与免疫功能,降低机体炎症反应,促进患者临床症状的缓解。

Objective To analyze the effects of Banxia Xiexin decoction on the clinical effect,immune function and serum interleukin-2(IL-2),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)in patients with chronic atrophic gastritis of spleen-stomach damp-heat type.Methods 160 patients with chronic atrophic gastritis of the spleen-stomach damp-heat type treated in the Sichuan Second Hospital of TCM from June 2017 to February 2021 were selected as the research objects,and which were divided into the control group and the research group with 80 patients in each group by random number table method.Patients in the control group were treated with conventional medications(Lansoprazole tablets,Amoxicillin capsules and Clarithromycin capsules),and patients in the research group were treated with Banxia Xiexin decoction quantitatively based on the control group,both groups were treated for 1 months.The clinical efficacy,main TCM syndrome score,percentage of CD3^(+),CD4^(+),CD8^(+)and ratio of CD4^(+)/CD8^(+),serum levels of IL-2,IL-6,TNF-α,and adverse reactions were compared between the two groups.Results After treatment,the total effective rate of the research group was 93.75%,which was higher than 73.75%of the control group;the percentage of CD3^(+),CD4^(+)and the ratio of CD4^(+)/CD8^(+)in the two groups were higher than before treatment,and the research group was higher than the control group;the TCM syndrome score,percentage of CD8^(+),serum levels of IL-2,IL-6 and TNF-αin the two groups were lower than before treatment,and the research group was lower than the control group(all P<0.05).Conclusion Combined therapy with Banxia Xiexin decoction can significantly improve the clinical efficacy and immune function of patients with chronic atrophic gastritis of spleen-stomach damp-heat type,reduce the inflammatory response of the body,and promote the relief of clinical symptoms,and with high safety.
