摘要
目的:探究胎膜早破孕妇胎膜组织中人类白细胞抗原G(HLA-G)、氧化型α1-抗胰蛋白酶(ox-AAT)表达及与基质金属蛋白酶-9(MMP-9)的相关性。方法:回顾性分析本院2020年1月-2021年1月收治的148例胎膜早破孕妇的临床资料,依据其分娩情况分为足月胎膜早破(A组,86例)与足月前胎膜早破(B组,62例),另选取同期正常足月妊娠的60例孕妇作为健康对照组(对照组),对比三组孕妇HLA-G、ox-AAT、MMP-9水平差异,采用Person相关性分析胎膜组织中HLA-G、ox-AAT表达与MMP-9水平的关系。结果:A、B组孕妇母血、脐血、胎膜组织中HLA-G水平显著低于对照组,水平由低至高依次为B组<A组<对照组;A、B组孕妇母血、脐血、胎膜组织中ox-AAT与MMP-9水平均显著高于对照组,水平由低至高依次为对照组<A组<B组(P<0.05)。Person相关性分析结果显示,HLA-G与MMP-9水平呈显著负相关(r=-0.117,P<0.05),ox-AAT与MMP-9水平呈现显著正相关(r=0.727,P<0.05)。结论:胎膜早破孕妇胎膜组织中的HLA-G、ox-AAT水平的变化与MMP-9相关性显著,能够将其作为临床判断孕妇胎膜早破及早产的可靠指标。
Objective:To explore the expression of human leukocyte antigen G(HLA-G)and oxidized-α1-antitrypsin(ox-AAT)in the fetal membrane tissue of pregnant women with premature rupture of membranes and their relationship with matrix metalloproteinase-9(MMP-9).Methods:The clinical data of 148 pregnant women with premature rupture of membranes admitted to our hospital from January 2020 to January 2021 were retrospectively analyzed.According to their delivery status,they were divided into term premature rupture of membranes(group A,86 cases)and term preterm premature rupture of membranes(group B,62 cases),another 60 pregnant women with normal term pregnancy in the same period were selected as the healthy control group(control group),and the differences in the levels of HLA-G,ox-AAT,and MMP-9 among the three groups of pregnant women were compared,and Person correlation was used analyze the relationship between the expression of HLA-G,ox-AAT and the level of MMP-9 in fetal membrane tissue.Results:The levels of HLA-G in maternal blood,cord blood,and fetal membrane tissues of pregnant women in groups A and B were significantly lower than those in the control group,and the levels from low to high were in order of group B<group A<control group.The levels of ox-AAT and MMP-9 in fetal membrane tissue were significantly higher than those in the control group,and the levels from low to high were in the order of control group<group A<group B(P<0.05).Person correlation analysis results showed thatHLA-G and MMP-9 levels showed a significantly negative correlation(r=-0.117,P<0.05),ox-AAT and MMP-9 levels showed a significant positive correlation(r=0.727,P<0.05).Conclusion:The changes of HLA-G and ox-AAT levels in the fetal membrane tissue of pregnant women with premature rupture of membranes are significantly correlated with MMP-9,which can be used as a reliable indicator for clinical judgment of premature rupture of membranes and premature delivery.
