胃苓汤加减对2型糖尿病脾虚痰湿证胰岛素抵抗的影响

Clinical observation of modified Weiling decoction on insulin resistance caused by type 2 diabetes mellitus with syndrome of spleen deficiency and phlegm dampness

目的:观察中药胃苓汤加减对2型糖尿病脾虚痰湿证患者胰岛素抵抗(IR)的影响。方法:将入选病例随机分为治疗组和对照组各30例,治疗组予中药胃苓汤加减治疗,对照组予盐酸二甲双胍片治疗,观察治疗前后临床症状、糖代谢(FPG、2hPG、HbA1c)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)水平变化情况。结果:治疗后两组患者的症状积分与治疗前比较均下降(P﹤0.05);且治疗组下降幅度优于对照组(P﹤0.05)。两组的血糖水平及胰岛素抵抗程度较治疗前均有一定程度的改善;治疗后治疗组FPG、2hPG、HbA1c及HOMA-IR水平下降幅度优于对照组(P﹤0.05)。结论:胃苓汤加减能显著改善患者的临床症状,且在一定程度上能调节患者的血糖水平及改善降低胰岛素抵抗。

Objective:To observe the clinical effect of Chinese materia medica modified Weiling decoction on insulin resistance(IR)caused by type 2 diabetes mellitus with syndrome of spleen deficiency and phlegm dampness.Methods:60 cases of selected patients were randomly divided into treatment group and control group,30 cases in each group.The treatment group was treated with Chinese materia medica modified Weiling decoction,while the control group was treated with oral metformin hydrochloride tablets.The changes of clinical symptoms,lucose metabolism(FPG,2hPG,HbA1c)and insulin(FINS)and insulin resistance index(HOMA-IR)were observed before and after treatment.Results:After treatment,the symptom scores of patients in two groups were lower than before treatment(P﹤0.05),and the decreasing amplitude of symptom score of treatment group was greater than that of control group(P﹤0.05).The level of blood glucose and the insulin resistance in two groups were improved to some extent comparede before treatment.The decreasing extent of FPG,2hPG,HbA1c and HOMA-IR levels of treatment group was better than that of control group after treatment(P﹤0.05).Conclusion:Modified Weiling decoction can significantly improve the clinical symptoms of patients,regulate the blood glucose level of patients to a certain extent,and improve the insulin resistance.
