呼吸与危重症医学科护士疲劳状况及与觉察压力、休闲满意度的关系

Fatigue status of nurses in department of pulmonary and critical care medicine and its relationship with perceived stress and leisure satisfaction

目的了解呼吸与危重症医学科护士疲劳状况及与觉察压力、休闲满意度的关系与可能存在的影响路径。方法 2020年9月—2021年1月随机选取解放军总医院呼吸与危重症医学科护士246例进行问卷调查,并对数据进行分析。结果呼吸与危重症医学科护士疲劳状况得分为58.80±9.36。骨干护士脑力疲劳得分高于非骨干护士,经常参加休闲活动护士疲劳总分及脑力疲劳得分均低于不参加或偶尔参加护士(均P<0.05)。职业规划较强护士疲劳总分及躯体、脑力疲劳得分均低于职业规划一般或无规划的护士(均P<0.05)。工作认知较低的护士疲劳总分及躯体、脑力疲劳得分均高于工作认知一般或较高护士(均P<0.01)。觉察压力、休闲满意度能显著预测呼吸与危重症医学科护士的疲劳状况(β=0.171、-0.273,均P<0.01)。休闲满意度在觉察压力和疲劳状况之间具有部分中介作用,占总效应的31.73%(0.119/0.375)。结论呼吸与危重症医学科护士疲劳状况具有不同的特点,与觉察压力、休闲满意度关系密切,休闲满意度在觉察压力和疲劳状况间具有中介作用。

Objective To explore the fatigue status of nurses in department of pulmonary and critical care medicine and its relationship with perceived stress and leisure satisfaction,analyze the possible impact paths. Methods From September 2020 to January 2021,246 nurses were randomly selected in Pulmonary and Critical Care Medicine Department of Second Medical Center of General Hospital of PLA to perform the questionnaire investigation,and the data were analyzed. Results The score of fatigue status of nurses in department of pulmonary and critical care medicine was 58.80 ±9.36. Mental fatigue score in backbone nurses were significantly higher than that in non-backbone nurses(P<0.05). The total score of fatigue and score of mental fatigue in nurses often take part in leisure activities were significantly lower than those in nurses do not often or occasionally take part in leisure activities(all P<0.05). The total score of fatigue and score of mental fatigue in nurses with strong career planning were significantly lower than those in nurses with general career planning or without career planning(all P<0.05). The total score of fatigue and score of physical fatigue and mental fatigue in nurses with low work cognition were significantly higher than those in nurses with moderate or high work cognition(all P<0.05). Perceived stress and leisure satisfaction could significantly predict the fatigue status among nurses in department of pulmonary and critical care medicine (β=0.171,-0.273,both P<0.01 ). Leisure satisfaction had a partial mediating effect between perceived stress and fatigue status,accounting for 31.73% of the total effect(0.119/0.375). Conclusion The nurses in department of pulmonary and critical care medicine have the fatigue status with different characteristics,which is closely related to perceived stress and leisure satisfaction,and leisure satisfaction has a mediating role between perceived stress and fatigue status.
