真实世界中银花泌炎灵片治疗急性泌尿系感染的联合用药复杂网络分析

Complex Network Analysis of the Combination of Yinhua Miyanling Tablet in the Treatment of Acute Urinary Tract Infection in the Real World

目的为分析真实世界中银花泌炎灵片治疗急性泌尿系感染时的联合用药药理规律。方法从全国16家三级甲等医院信息系统中提取出银花泌炎灵片治疗急性泌尿系感染的医嘱数据,将联合用药按照药理进行整理合并,标准化后再用Tabu搜索算法分析潜在规律。结果急性泌尿系感染的好转、治愈人群,应用银花泌炎灵片时常联合使用的中西药理方案有4种:1)联合通淋排石剂、清热利湿剂、抗真菌药、益气剂、解表剂;2)联合尿路解痉药、止血药、补肝肾剂;3)联合清热剂、解热镇痛抗炎药、补益剂、化湿剂、健脾益肾剂;4)联合抗生素类、活血化瘀剂。西药药理选择以抗生素为主,中药药理常见组合为清热剂、化湿剂、补肝肾剂。结论临床治疗急性泌尿系感染多用西药抗生素,中药以清热利湿剂为主,兼补肝肾,与病因、指南推荐、中医淋证基本病机等相契合。体现出中西医联合治疗的优势,可为临床优化处方药理分布提供一定思路参考,同时需根据病情和相关并发症进行个体化用药。

Objective To analyze the pharmacological regularity of combined medication in the treatment of acute urinary tract infections in Yinhua Miyanling tablets in the real world.Methods In this study,the medical order data of Yinhua Miyanling tablet in the treatment of acute urinary tract infection were extracted from the information system of 16 third-class first-class hospitals in China.The combined drugs were sorted and combined according to pharmacology.After standardization,the potential online cables were analyzed by Tabu search algorithm.Results There are 4 kinds of Chinese and Western pharmacological schemes commonly used in combination with Yinhua Miyanling tablets in the cured and cured people with acute urinary tract infection.1)Combined Tonglin stone removal agent,heat clearing and dampening agent,antifungal agent,Yiqi agent,Anti-surface agents;2)combined with urinary tract antispasmodic drugs,hemostatic drugs,liver and kidney agents;3)combined with heat-clearing agents,antipyretic and analgesic anti-inflammatory drugs,tonic agents,humectants,Jianpi Yishen agents;4)combined antibiotic,Huoxue Huayu agent;Western medicine pharmacological choice is mainly based on antibiotics.The common combination of Chinese medicine pharmacology is heatclearing agent,humidifying agent,liver and kidney agent.Conclusion The clinical treatment of acute urinary tract infections mostly uses western medicine antibiotics.The traditional Chinese medicine is mainly clearing heat and dampening agents,and supplements the liver and kidney.Reflecting the advantages of combined Chinese and Western medicine treatment,it can provide a reference for clinical optimization of the pharmacological distribution of prescriptions,and individualized medication according to the condition and related complications.
