马应龙痔疮软膏治疗中风病后Ⅰ、Ⅱ期压疮的临床观察

Efficacy of Mayinglong Hemorrhoids Ointment in the Treatment of StageⅠandⅡPressure Sore after Stroke:A Randomized Controlled Trial

目的探讨马应龙痔疮软膏治疗中风病后Ⅰ、Ⅱ期压疮的临床疗效。方法选取2013年3月—2018年2月期间北京中医药大学东直门医院脑病科收治的中风病后Ⅰ、Ⅱ期压疮患者84例,根据随机数字表法分为干预组与对照组,每组各42例。干预组消毒或清创后予马应龙痔疮软膏治疗,对照组予利凡诺治疗。两组患者均连续治疗14 d后,以临床疗效中治愈率为主要疗效指标,以压疮愈合评价量表(Pressure ulcer scale for healing,PUSH)、压疮愈合时间和治疗满意度为次要疗效指标评价治疗前后两组患者的疗效及满意度。结果治疗后干预组治愈率为77.78%,对照组治愈率为52.83%,干预组优于对照组,两组比较,差异有统计学意义(P<0.05)。治疗后两组中风后压疮患者PUSH量表评分均较治疗前明显降低,差异有统计学意义(P<0.01);且干预组治疗后PUSH量表评分明显低于对照组,两组比较,差异有统计学意义(P<0.01)。治疗后干预组压疮创面愈合时间优于对照组,两组比较,差异有统计学意义(P<0.01)。治疗后干预组满意率为97.62%,对照组满意率88.10%,干预组满意度明显高于对照组,两组比较,差异有统计学意义(P<0.01)。结论马应龙痔疮软膏在治疗中风后Ⅰ、Ⅱ期压疮的疗效确切,患者依从性与满意度良好,适宜临床推广应用。

Objective To investigate the clinical efficacy of Ma Yinglong Hemorrhoids Ointment in the treatment of stageⅠandⅡpressure ulcers after stroke.Methods 84 patients with stageⅠandⅡpressure sores after stroke treated in the Department of encephalology,Dongzhimen Hospital,Beijing University of traditional Chinese medicine from March 2013 to February 2018 were randomly divided into intervention group and control group,with 42 cases in each group.The intervention group was treated with Ma Yinglong Hemorrhoid Ointment after disinfection or debridement,and the control group was treated with rivanol.Both groups were treated for 14 days.The cure rate in the clinical efficacy of pressure ulcer treatment was the main efficacy index,and the pressure ulcer healing evaluation scale,pressure ulcer healing time and treatment satisfaction were the secondary efficacy indexes to evaluate the efficacy and satisfaction of the two groups before and after treatment.Results After treatment,the cure rate of the intervention group was 77.78%,and the cure rate of the control group was 52.83%.The intervention group was better than the control group.The difference between the two groups was statistically significant(P<0.05).After treatment,the PUSH scale scores of patients with pressure ulcers after stroke in the two groups were significantly lower than those before treatment,and the difference was statistically significant(P<0.01);and the PUSH scale scores of the intervention group after treatment were significantly lower than those of the control group.The difference was statistically significant(P<0.01).After treatment,the pressure sore wound healing time of the intervention group was better than that of the control group,and the difference between the two groups was statistically significant(P<0.01).After treatment,the satisfaction rate of the intervention group was 97.62%,and the satisfaction rate of the control group was 88.10%.The satisfaction rate of the intervention group was significantly higher than that of the control group.The difference between the two groups was statistically significant(P<0.01).Conclusion Mayinglong hemorrhoids ointment is effective in the treatment of stageⅠandⅡpressure sore after stroke,with good compliance and satisfaction,which is suitable for clinical application.
