低温等离子射频消融术治疗早期声门型喉癌远期疗效观察

Long-term treatment efficiency of low temperature plasma radiofrequency ablation for early glottic carcinoma

目的探讨低温等离子射频消融技术治疗早期声门型喉癌的临床意义。方法回顾性分析2012年4月~2015年6月收治的40例声带癌患者临床资料和随访结果。左侧22例、右侧17例,双侧1例,电子喉镜、磁共振检查评定为T1/T2病变,无颈部淋巴结转移,全麻插管、应用低温等离子射频消融手术,完全切除肿瘤,术中快速病理切缘阴性。术后常规治疗,定期随访,评价5年疗效。结果所有患者一次手术完全切除病变,出血少(5~30ml),术后拔管顺利,无喉梗阻发生,无气管切开,无鼻饲,术后恢复良好,平均住院时间7天。随访5年无复发,局部恢复良好。结论低温等离子射频消融手术治疗早期声门型喉癌创伤小,手术安全性较高、疗效好,是治疗的良好选择。

Objective To explore the clinical value of plasma radiofrequency ablation in the treatment of malignant laryngeal lesions and improve the clinical efficacy.Methods A retrospective studies on clinical data and follow-up results of 40 patients with vocal cord cancer admitted to our department from April 2012 to June 2015 was analyzed.There were 22 leisons on the left vocal cord and 17 cases on the right vocal cord,and 1 leison on both vocal sides.The lesions were assessed as T1 by electronic laryngoscopy and magnetic resonance examination,without cervical lymph node metastasis.Postoperative routine treatment,follow-up was completed until 5 years.Results All lesions were completely resected at one time with less bleeding(5~30ml),and the postoperative extubation was successfully,without laryngeal obstruction,tracheotomy or nasogastric feeding,and the postoperative recovery was good.The average hospital stay was 7 days,no recurrence was observed until 5-years followed-up periods.Conclusion Low temperature plasma radiofrequency ablation is a good choice for the treatment of early glottic laryngeal cancer with less trauma,less pain,higher surgical safety,better curative effect and shorter hospital stay.
