摘要
目的:观察床边经腹彩色多普勒超声引导下介入结合抗菌药物注药冲洗治疗细菌性肝脓肿的临床效果。方法:选取平顶山第五人民医院2017年1月至2019年3月接诊治疗的细菌性肝脓肿急诊患者72例为研究对象,采用随机数字表法分为对照组(36例)和观察组(36例)。对照组给予腹腔镜引流术及手术前后抗感染治疗,观察组给予患者经腹彩色多普勒超声导引下行穿刺抽脓、抗菌药物注药冲洗及置管引流。结果:观察组患者临床总有效率(100.00%)及并发症发生率(0.00%)均明显优于对照组(75.00%,2.80%),差异具有统计学意义(P<0.05)。结论:床边经腹彩色多普勒超声除了可以提高术野清晰度,帮助医师掌握病情,还能通过超声引导下经皮肝穿刺细针抽吸、注药冲洗、置管引流技术介入治疗,手术操作简单且安全可靠。
Objective To observe the clinical effect of bedside transabdominal color Doppler ultrasound guidance in the treatment of bacterial liver abscess by intervention combined with antibiotic injection and flushing.Methods A total of 72 emergency patients with bacterial liver abscess who received treatment from Pingdingshan Fifth People's Hospital from January 2017 to March 2019 were selected as the research objects,and they were randomly divided into a control group(36 cases)and an observation group(36 cases).The control group was given laparoscopic drainage and anti-infective treatment before and after the operation.The observation group was given the patients with abdominal color Doppler ultrasound guided downward puncture and pus extraction,antibiotic injection and flushing,and catheter drainage.Results The total clinical effective rate(100.00%)and complication rate(0.00%)of the observation group were significantly better than those of the control group(75.00%,2.80%),and the differences were statistically significant(P<0.05).Conclusion The bedside transabdominal color Doppler ultrasound can not only improve the clarity of the operation field and help the doctor grasp the condition,but also use ultrasound-guided percutaneous liver puncture with fine needle aspiration,drug injection and irrigation,and catheter drainage technology for interventional therapy.The operation is simple,safe and reliable.
