环磷酰胺结合糖皮质激素治疗非特异性闭塞性脑动脉炎的效果分析

Analysis of the Efficalcy Cyclophosphamide Combined with Glucocorticoid in the Treatment of Non-Specific Cerebral Arteritis Obliterans

目的:探讨环磷酰胺结合糖皮质激素治疗非特异性闭塞性脑动脉炎的效果。方法:选取2018年1月至2019年12月在辽宁省健康产业集团抚矿总医院进行治疗的163例非特异性闭塞性脑动脉炎患者,随机分为单一组和联合组,单一组患者82例,联合组患者81例。单一组采取糖皮质激素进行治疗,联合组采取环磷酰胺结合糖皮质激素进行治疗,观察比较两组患者不良反应发生率、炎症反应因子水平、氧自由基以及治疗有效率。结果:联合组患者的不良反应发生率为18.52%,明显低于单一组的39.02%,差异具有统计学意义(P<0.05)。联合组患者的治疗有效率为69.14%,明显高于单一组的52.44%,差异具有统计学意义(P<0.05)。联合组患者的细胞间黏附分子–1(ICAM–1)、髓过氧化物酶(MPO)、超氧化物歧化酶(SOD)、丙二醛(MDA)等各项炎症反应因子以及氧自由基的水平均优于单一组,差异具有统计学意义(P<0.05)。结论:在非特异性闭塞性脑动脉炎治疗时,应用环磷酰胺结合糖皮质激素,能够减少不良反应,提高疗效,降低脑血管炎症因子水平。

Objective To explore the efficacy of cyclophosphamide combined with glucocorticoid in the treatment of nonspecific cerebral arteritis obliterans.Methods 163 cases of non-specific cerebral arteritis obliterans treated in Fukuang General Hospital of Liaoning Health Industry Group from January 2018 to December 2019 were selected and divided into single group and combination group,with 82 patients in single group and 81 patients in combination group.The single group was treated with glucocorticoids,and the combination group was treated with cyclophosphamide combined with glucocorticoids.The incidence of adverse reactions,inflammatory response factors,oxygen free radicals and the effective rate of treatment were observed and compared between the two groups.Results The adverse reaction rate of patients in the combination group was 18.52%,which was significantly lower than 39.02%in the single group,and the difference was statistically significant(P<0.05).The treatment effective rate of patients in the combination group was 69.14%,which was significantly higher than 52.44%in the single group,the difference was statistically significant(P<0.05);the intercellular adhesion molecule-1(ICAM-1),myeloperoxidase(MPO),superoxide dismutase(SOD),malondialdehyde(MDA)and other inflammatory response factors and oxygen free radical levels in the combination group were better than the single group,the difference was statistically significant(P<0.05).Conclusion In the treatment of non-specific occlusive cerebral arteritis,the application of cyclophosphamide combined with glucocorticoids can reduce adverse reactions,improve curative effects,and reduce the level of cerebrovascular inflammatory factors.
