对某院脾胃病科不同病种证型使用中药饮片的用药分析

Analysis of the Use of Chinese Herbal Pieces in Different Types of Diseases and Syndrome in the Department of Spleen Disease and Gastropathy in a Hospital

目的:通过对本院脾胃病科不同病种证型使用中药饮片情况进行统计分析,归纳病、证、方药间的关系,以期为临床合理用药和药师审核处方提供参考。方法:选取本院2018年1月~2019年5月期间脾胃病科所开具的中药处方,统计分析不同病种、不同中医证型的中药处方中使用频次最高的中药饮片及其作用分类、用药频次、常用剂量。结果:共纳入处方16386张。其中,最常见疾病为痞满(3480例,33.96%),使用最多的中药饮片为白术(1766例次,50.75%);最常见中医证型为脾胃不和证(2370例,16.62%),使用最多中药饮片为白术(1324例次,55.86%);不同病种与不同证型交叉分析可得,治疗脾胃病多用理气药和补气药,使用最多中药饮片为白术,常用陈皮、茯苓、香附、柴胡。结论:脾胃病科疾病治疗应以补脾气、降胃气、健脾燥湿、清胃热为主要原则,通过辨证、分析疾病的证候确定具体治疗方案。

Objective:Through the statistical analysis of the use of Chinese herbal pieces in different types of diseases in the department of spleen and stomach in our hospital,the relationship among diseases,syndromes and prescriptions was summarized,so as to provide references for clinical rational drug use and pharmacist s prescription review.Methods:Select the traditional Chinese medicine prescriptions issued by the department of spleen and stomach in our hospital from January 2018 to May 2019,and statistically analyze the drugs with the highest frequency in the traditional Chinese medicine(TCM)prescriptions of different diseases and different Chinese herbal piece syndrome types,their action classification,medication frequency and common dose.Results:A total of 16386 prescriptions were included.Among them,the most common disease was Piman(3480 sheets,33.96%),and the most used Chinese herbal piece was Atractylodis macrocephalae Rhizoma(1766 cases,50.75%).The most common TCM syndrome type was disharmony between spleen and stomach(2370 cases,16.62%),and the most used Chinese herbal piece was Atractylodis macrocephalae Rhizoma(1324 cases,55.86%).According to the cross analysis of different diseases and different syndrome types,Qi regulating drugs and Qi tonifying drugs are often used in the treatment of spleen and stomach diseases.Atractylodis macrocephalae Rhizoma is the most used Chinese herbal piece,and Citri Reticulatae Pericarpium,Poria,Cyperi Rhizoma,Bupleuri Radix are commonly used.Conclusion:The treatment of diseases in the department of spleen and stomach is mainly based on the principle of repair tempers,down the stomach,fortify the spleen and dry dampness and clearance stomach hot.The specific treatment plan is determined by syndrome differentiation and analysis of disease syndromes.
