不同甲状旁腺自体移植区域对甲状腺癌根治术后甲状旁腺素的影响

Effect of different parathyroid autotransplantation areas on parathyroid hormone after radical thyroidectomy

【目的】探究不同区域的甲状旁腺自体移植术对完全腔镜下甲状腺癌根治术后甲状旁腺功能恢复的影响。【方法】收集2020年1-10月于我院接受经腔镜下行甲状腺癌根治术及甲状旁腺自体移植术的患者60例,根据甲状旁腺自体移植区域的不同分为胸锁乳突肌组与胸大肌组,各30例。对比患者术前及术后第1天、1个月、3个月检测血清钙及甲状旁腺激素水平。【结果】两组患者术前、术后第1天、1个月、3个月,患者甲状旁腺激素(PTH)水平均呈下降趋势,组内时间点差异均有显著性(P<0.05),两组术后同时间点PHT水平、术后并发症差异无显著性(P>0.05)。【结论】胸锁乳突肌和胸大肌作为甲状旁腺自体移植区对完全腔镜下甲状腺癌根治术后甲状旁腺功能恢复不具有影响,术后3个月甲状旁腺均可基本恢复正常功能。

【Objective】To investigate the effect of parathyroid autotransplantation in different regions on the recovery of parathyroid function after complete endoscopic radical thyroidectomy.【Methods】60 patients who underwent endoscopic radical thyroidectomy and parathyroid autograft in our hospital from January to October 2020 were collected.According to the different areas of parathyroid autograft,they were divided into sternocleidomastoid muscle group and pectoralis major muscle group,with 30 cases in each group.The levels of serum calcium and parathyroid hormone were measured before operation and 1 day,1 month and 3 months after operation.【Results】The level of parathyroid hormone(PTH)in the two groups decreased before operation,on the first day,one month and three months after operation.There was significant difference at the time point in the group(P<0.05).There was no significant difference in PHT level and postoperative complications between the two groups at the same time point after operation(P>0.05).【Conclusion】Sternocleidomastoid muscle and pectoralis major muscle as parathyroid autotransplantation area have no effect on the recovery of parathyroid function after complete endoscopic radical thyroidectomy.The parathyroid function can basically return to normal 3 months after operation.
