阴道产钳助产对初产妇产后盆底功能、性心理的影响

Effects of Forceps Assisted Vaginal Delivery on Postpartum Pelvic Floor Function and Sexual Psychology in Primiparas

目的:探讨阴道产钳助产对初产妇产后盆底功能、性心理的影响。方法:选取2019年2月-2021年2月在我院初次分娩的80例产妇,根据分娩方式分为阴道自然分娩组(36例)和阴道产钳助产组(44例)。观察两组产妇的第二及第三产程时间,于产后12周、16周检测两组产妇的盆底肌力和阴道肌张力;采用女性性功能评估量表(BISF-W)评估两组产妇性功能;观察两组产妇产后并发症发生率。结果:自然分娩组的第二产程时间短于产钳助产组(P<0.05)。产后16周,产钳助产组的Ⅰ类肌纤维和Ⅱ类肌纤维的肌电值、阴道肌张力低于自然分娩组,疲劳度、性功能评分高于自然分娩组(均P<0.05)。产钳助产组的产后并发症发生率高于自然分娩组(P<0.05)。结论:阴道产钳助产对产妇产后盆底肌功能和性功能的损伤较大,临床上应尽量避免使用阴道产钳助产,对无法避免使用的产妇产后应注意盆底肌功能和性功能的恢复。

Objective:To explore the effects of forceps assisted vaginal delivery on postpartum pelvic floor function and sexual psychology of primiparas.Methods:A total of 80 primiparas in our hospital between February 2019 and February 2021 were enrolled.According to different delivery modes,they were divided into the natural vaginal delivery group(36 cases)and the forceps assisted vaginal delivery group(44 cases).Duration of the second and third stages of labor were recorded.On 12 and 16 weeks after delivery,pelvic floor muscle strength and vaginal muscle tension were detected.The sexual function was evaluated using brief index of sexual function for women(BISF-W).The incidence of postpartum complications was compared.Results:The second stage of labor in the natural vaginal delivery group was shorter than that in the forceps assisted vaginal delivery group(P<0.05).On 16 weeks after delivery,myoelectric voltages of type I and II muscle fibers,as well as vaginal muscle tension in the forceps assisted vaginal delivery group were lower,while scores of fatigue and sexual function were higher,when compared with the natural vaginal delivery group(all P<0.05).The incidence of postpartum complications in the forceps assisted vaginal delivery group was higher than that in the natural vaginal delivery group(P<0.05).Conclusion:Forceps assisted vaginal delivery can cause great damage to the pelvic floor muscle function and sexual function,so the use of vaginal forceps should be avoided as far as possible in clinical practice.For the parturient who cannot avoid using vaginal forceps,attention should be paid to the rehabilitation of pelvic floor muscles and sexual function.
