摘要
目的探究血清尿酸水平与不同糖代谢水平老年人体脂水平之间的相关性。方法选取2017年3月-2020年12月于同济大学附属上海市第四人民医院进行糖尿病检查的老年人488例,根据糖尿病诊断标准分为正常健康组(WT组),糖耐量异常组(IGR组)以及2型糖尿病组(T2DM组),比较3组的一般资料、体脂水平等,分析血尿酸与体脂水平的相关性。结果在WT组中,无高尿酸血症者与高尿酸血症者在BMI、TG、HDL-C、LDL-C和空腹胰岛素水平上有差异(P<0.05),在IGR组中,无高尿酸血症者与高尿酸血症者在性别,BMI、TG、TC、HDL-C和LDL-C水平上有差异(P<0.05);在T2DM组中,无高尿酸血症者与高尿酸血症者在性别,BMI、TC和LDL-C水平上有差异(P<0.05)。在WT组,无高尿酸血症者与高尿酸血症者在TF%/LF%、TFM/LFM和A/G水平上有差异(P<0.05);在IGR组,无高尿酸血症者与高尿酸血症者在腰臀比,TF%/LF%、TFM/LFM和A/G水平上有差异(P<0.05);在T2DM组,无高尿酸血症者与高尿酸血症者在BF%、TF%水平上有差异(P<0.05)。在WT组与IGR组,血尿酸水平与BMI、腰臀比、TF%/LF%、TFM/LFM以及A/G成正相关,与BF%以及TF%呈负相关(P<0.05);在T2DM组,血尿酸水平与BMI、TF%/LF%、TFM/LFM及A/G成正相关(P<0.05)。结论不同糖代谢水平的老年人的血尿酸水平及体脂水平存在差异,血尿酸水平的高低与糖代谢水平及体脂分布密切相关。
Objective To explore the correlation between the blood uric acid level and the body fat level in the elderly with different glucose metabolism levels.Methods A total of 488 elderly patients who underwent diabetes examination in Fourth People’s Hospital of Shanghai City from March 2017 to December-2020 were selected.According to the diagnostic criteria of diabetes,they were divided into normal healthy group(WT group),impaired glucose tolerance group(IGR group)and type2 diabetes group(T2DM group).The general data and body fat level of the three groups were compared,and the correlation between blood uric acid and body fat level was analyzed.Results In the WT group,there were differences in the levels of BMI,TG,HDL-C,LDL-C and fasting insulin between those without hyperuricemia and those with hyperuricemia(P<0.05).In the IGR group,there were differences in gender and the levels of BMI,TG,TC,HDL-C and LDL-C between those without hyperuricemia and those with hyperuricemia(P<0.05).In the T2DM group,there were differences in gender and the levels of BMI,TC and LDL-C between those without hyperuricemia and those with hyperuricemia(P<0.05).In the WT group,there were differences in TF%/LF%,TFM/LFM and A/G between those without hyperuricemia and those with hyperuricemia(P<0.05).In the IGR group,there were differences in waist-to-hip ratio,TF%/LF%,TFM/LFM and A/G between those without hyperuricemia and those with hyperuricemia(P<0.05).In the T2DM group,there were differences in BF%and TF%between those without hyperuricemia and those with hyperuricemia(P<0.05).In the WT group and IGR group,blood uric acid level was positively correlated with BMI,waist-to-hip ratio,TF%/LF%,TFM/LFM and A/G,and negatively correlated with BF%and TF%(P<0.05).In the T2DM group,blood uric acid level was positively correlated with BMI,TF%/LF%,TFM/LFM and A/G(P<0.05).Conclusion The level of blood uric acid and body fat in the elderly with different glucose metabolism levels are different.The level of blood uric acid is closely related to the level of glucose metabolism and body fat distribution.
